Question Scrotal pain next step Mx

56 yrs old patient came with severe scrotal pain , fever. On examination he has tender cord, normal position of testis with exquisite testicular pain ..... what is next step in management ....
1. Antibiotics
2. Surgery
3. Doppler ultrasound
4. Conservative
5. Urine analysis

i think it is urine analysis because cases of epidydimorchitis usualy comes after urinary infection

Maybe we should start with empirical antibiotics as we wait for urine culture results

Its doppler u/s ..... to exclude torsion before proceeding for epididymo orchitis , as torsion is an emergency .......

torsion testis not present at all at this old age

Yup its my mistake , age is 16 , i read but didnt notice age , age is really important factor always occurs in young people .... Thanks to show me my mistake ...... I think now next step in epididymo orchitis is doppler u/s .... Epididymo orchitis also uncommon at that age , its peak with 19 to 35 yrs ..........
