Default surgery q- retroperitoneal hematoma contraindication?

A 30-year-old man is brought by ambulance to
the emergency department after a major motor
vehicle crash. He is lethargic but conscious
and after the primary survey he is found to
have signifi cant hypotension and tachycardia.
Secondary survey reveals an unstable pelvic
girdle and what appears to be a developing pelvic
hematoma. There is no blood at the urethral
meatus. Anteroposterior x-ray of the pelvis
is shown in the image. Which of the following
is contraindicated in this patient?
(A) Emergent pelvic angiography
(B) Exploration of the hematoma
(C) Manual prostate examination
(D) Orthopedic consultation for fi xation
(E) Retrograde urethrogram
(F) Volume resuscitation

Correct Answer

B exploration of the hematoma.

With an unstable pelvic girdle, exploration would worsen the condition.
Fluid resuscitation and external fixation are the best options at this point.
