Default young pt with confusion

A 26-year-old female is brought to the emergency room by her friends who say she is “acting weird”. Her friends tell you that she has a blood disorder, but they do not know her medical condition very well. They say that over the last day she has been confused and slightly agitated. On questioning she complains of a headache. She knows who she is and what year it is but is unable to name the month or the day of the week. Her vital signs are pulse 101 beats/min, blood pressure 155/88 mm Hg, respiratory rate 16 breaths/min, and oxygen saturation 95% on room air. She moves all 4 extremities and follows simple commands. Her laboratory results show a platelet count of 15,000/μL and a normocytic anemia. What treatment does she need?

A. Antibiotics
b. exchange transfusion

c plasma exchange
d. dialysis

may i know what is the difference between exchange transfusion and plasma exchange?
