Embarrassed Old mom journal

Hey there,
I am 95 graduate, from pakistan, I did my step 1 last year with just 80%, 196 score, I am preparing for step 2 now!
Why I am thinking about my career now?? I spent all this time to take care of kids, I have so many doubts daily, that I should not even try, its going to be really hard plus I have no clinical experience, I am like living in my cocoon for so long... but than someone inside me say, no you can do it! I need to work for my kids good future...
I am very desperate, and sad!
People make fun of me...but I am determined, I will show them...if some how I ace in step 2...I am looking for all the support!
I found this forum, it seems really nice!
I have once watched all the videos of im, surgery, now watching gyn and obs, did uw once except gyn/obs qs!

I am really looking forward to this community!

I know its awkward to post my daily accomplishments, but I have nobody to talk to, except this place!

I have done 3 more hrs of videos and 11 qs, its hard to retain!

TWo important point: never ever give ACE or ARB to pregnant women!
Pre-menopausal woman with LSIL go ahead with colposcopy!

  • LEEP, Cone biopsy or DES causes cervical insuffeciency
  • Evaluate with t/v us
  • 23-28 weeks cevix should be 25 mm,
  • No philtrum+thin upper lip-->maternal alcohol syndrome
  • Tamoxifen--> causes end ca
  • Raloxifen: rx for osteoporosis, can cause DVT,bisphosphonates are better.
  • Early deceleration-->fetal head compression
  • Late deceleration--> utero-placental insufficiency


Just did 15 more uw qs!

I am really slow
but, I waste alot of time on face book
Got to fix myself! Its for my kids!

Ok made flash cards of preventive medicine, and read 20 pages of endocrinology kaplan physiology!
Todays progress is nod bad,
3 hrs of lecture+ 25 qs+preventive medicine+20 pages physio


I am happy I am really glad that I found this forum, please tell me, if I m doing it all wrong, I have to have good score...I dont know, I should leave it to God almighty!


  • 3 cancers should be screen CBC -->colon, breast and cervix
Pre travel screening:
  • Hav+IG+6months booster dose, 10 years of immunity.
  • Malaria prophylaxis is little bit confusing:
  • Chloroquine, if chloroquine resistance give meflaquine or Doxycycline, but doxy can cause photo sensitivity
  • If you are pregnant and you want to travel to mexico have a combo of atovaquine+proguanil
  • But if you are going to mexico or india where mosquito are living with dogs and skunks and you are afraid of rabies, you should take its vaccine, but not i/d, you should take it i/m bec intradermal vaccine of rabies will be in effective because of chloroquine!..hmmm
  • Travelers with bloody diarhea rx is flouroquinolone or azithromycin


  • pregnant +influenza season take vaccine in 2nd or 3rd trimester!

Hi everyone,
I am back here, I am almost done with gyn videos, will start obs soon.
Read the endo from kln, and read peds endo too.
Done 60 qs endo, and 3 blocks of gyn+obs!
Its not bad, right?

I am trying to remember things, lets see!

Primary hyperaldosteronism,
initial diagnostic test:
PA:PRA
How you should do it?
First if the pt is on bb,spironolactones, he should discontinue it for 2-4 weeks, than you should fix the hypokalemia before doing the test, this test should be done early morning, if the ratio comes more than 30, than you should do the confirmatory test.

And the confirmatory test is iv nacl, if still aldosterone is high, which is more than 14 than it is confirmed, but now you have to locate the problem, so do the ct scan, if you cant see on ct, do the adrenal vein sampling!

  • RA is very rare in HIV, bec RA needs T cells, HIV has low T cell!
  • DIP and joints of lower back are NEVER involved in RA.
  • When the Fingers Are Abnormally Bent

  • Rheumatoid nodules are small bumps under the skin, located in pressure point areas such as the elbow, back of the hand, or back of the heel. These bumps may be as small as a grain of rice or as large as a golf ball, but they are usually not painful and they tend to come and go.
  • Rh nodules may flare up by methotrexate.

RA + pneumoconiosis --> Caplan syndrome

Also, although the DIP are never affected with RA in the USMLE, in real life DIP involvement does not rule out RA

Any suggestions, how to go with the prep, I watched all the videos except obs, and psychiatry, and done uw once, but feel like I dont have any memory left,

Hi Drsaaz, I am new to the forum and I just read your first post ! Me too I am in the same boat like yours almost, I have 2 young kids ,i am 2008 graduate, I really lost all my knowledge since I ve been a housewife for 4 years! Between cooking, cleaning, nursing, and changing diapers I really feel shy to tell people that I am actually a foreign MD, I really do . Most people that I met down here don't know this fact, because once they know,they start asking questions " how come that u re a doctor , and bla bla bla,............."you must be experiencing same thing, I am sick and tired of it
I ve been in and out for the preparation for the exam, every time I start I give up very soon! Thanks God now I joined this fabulous community, getting answers from everyone and having such good ears for my concerns makes me so excited and wanting to go all the way to the exam if god will
As u said Drsaaz, I want to do it and especially for my kids, May God give me the strength to finish it all the way ! I want them to be proud of their moms!
I am not sure if I will get a residency somewhere because As I said earlier I had 4 years after my graduation just blank without any clinical experience so i dont think i will attractive for any hospital it is so critical but I want to get ECFMG at least , there will be lot of interesting things to do with in this country
I don't have a date for the exam yet , I started with the anatomy and I am still stuck there , I don't have a good routine yet
You re done with the step 1 , what a relief!! U were planning to do step 2 sometimes in may, r u still going for it??

Hi there,
I was away as I stopped preparing for ck for 2 months, because I was practicing CS cases, I am done with my CS, hoping for the best!
Now it is CK time, I have to give ck by July 30th, end of my eligibility...lets see...so I am here again, requesting for your support!
This forum is awesome, I am so sorry for not being here, as I had to be in library for CS practice, but now its summer break, my kids are home, cant go to library...
I don't know what is the fate for this hard work, but may God accept all the efforts!

I am doing quick read of peds today from MTB 3, feels like I forgot everything... It is a lot to study, just for one month....I will give NBME in mid of june, lets see!!

Its been really tough, and more tough times are coming, I don’t if I will be able to give exam in july or not, but I will definitely try if God wills.
I have divided my schedule according to uw qs, and it will take this whole month of june, oooooo boy!!
I wonder how I will do it?? So so so
The smart thing would be..hmmm…I think just hard work!!
Just need lots of prayers, no time to waste!!
Will be coming here on and off, only place where I can discuss my situation!

I am also a IMG and a mom now of a hermoso 7 months old baby... I would like to know if you can give me some suggestion in regards how to prepare to the step 1 base on your experience...

Hi hillarodri,

Kaplan is important, do 3 times kaplan, 2 times uw within one year, u will ace it.
If u can read Kaplan with FA.Add ur notes of kln and uw in FA, it will be easier to review in last.
I personally liked videos, for old people like me, and imgs...but its up to you!
Last but definitely not least, our children need their moms, more than this society needs doctors, and at age 7 months, he must be starting some separation anxiety...just a reminder to myself first...our first priority is our children!
Motherhood is so awesome!
Everything else comes second

Im a 2004 graduate from karachi ,pakistan.Dr.Saaz is right motherhood is awsome and children are our first priority.now that my kids are school going ,i can studyalso i believe im a better manager now(study and take care of house and kids).it took me 4-5 years to understand what i really wanted to do.i had severe phobias about usmle ,that it is the toughest exam in the world and impossible for me to do itinitially i thought i have wasted a seat but after performing hajj last year ,i truly understood to what a noble profession i belong to(Alhamdulillah).i started preparing for usmle step 2, 5 months ago and going to give the test inshallah in july next month.this community is really helpful,i wish everybody best of luck.i have stayed focused all these months ,overcame my exam fear.i think no exam is piece of cake but if you strive hard its not impossible either.i tried my best and ask Allah Almighty for help.lets see what happens in the test.

Arrow venting... need advice please.

I am just so worried about everything right now, its kind of like generalized anxiety disorder, every week there is some wedding, where we have to go..that means, that whole day I cant study, than so many distractions, I am worried about kids too, they are home in summer, so they need my time, and if I give them time, I cant study enough...means I cant study enough to give my exam in july…but I can study good 5-6 hrs!
I have to give one nbme...then I will see, I am loosing my focus, so much partying, than cooking, time management of kids, then loneliness...then I have anxiety of cs result too,
Sometimes it feel like I can still give my exam in july, but since last Wednesday(cs result is delayed), I couldn't study, my concentration is gone...
I really really want to give my exam in july, but without proper prep, I shouldnt...I should delay it, till I m ready, like in August, but giving exam in August will make my application delayed, which means I can only apply in next year, so I don't know what to do...then another thought pops up, that may be I should apply next year, because being so old, with 15 + years gap, and no usce, who would give me residency this year, so may be I should delay my exam, get good scores, get some good usce, and apply next year...may be this is the right strategy...may be!
Than this what if question pops up, what if I can still get residency this next year, what if I should just work really hard, and give my exam in july, get one or two months of usce and apply this year, may be I still have a chance...

I don't know I am so frustrated

So you are done ... I have not been able to sit and really start the journey... I have been always paralyzed by the fact that i will fail....plus now me and my husband have big issues with the visa and this is taking all my energy and effort to keep fighting....i wish I can try but so scare of failure....

Any advice are welcome....

Hello everyone,
This is me old mom...getting older, haven’t give my exam yet!
There were few family problems, health issues and others...so i had this huge gap,and I am on the same spot again, where I was last year!!
It is so depressing, but I cant afford to be depressed anymore, I got to do it, no option!
So, I know you must be thinking that, what in the world this crazy old fmg/mom/psychotic person is thinking.. I am this sad sad thing right now, who is on the verge of breaking down...and feeling horrible about myself...
But if you are struggling due to any kind of situation, may be you have ADHD like me, or you are working and its hard for you to get some time, or any other hurdle you are facing...please join me here...I believe that if I spread some positivity here, I will be positive for myself!
I WANT TO SURROUND MYSELF AND YOU IN HOPE AND INSpIratION!!
OK...

So...now let me come back to the CK!!

I got to start with HY Kaplan videos with MTB2 + FA Peds +Surgery Kaplan +UW notes + UW
10 hrs/day

I will update here daily...I should,right? or may be when I am in my manic phase I will be writing here hourly!!
I am starting right now, Immuno/allergic disorders! If you are online, write something, say something please, I need all the encouragement

I am scratching my head and my head is spinnig now. I am little bit ashamed also. I graduated in 2003 finished internship 2004. I was of the view that there is no door open for me in usmle. But after reading your post I am thinking again about usmle. Thankyou.
I have a family and I am working in a university healthcentre for 8 years and now I will seriously think about my usmle. Have you taken any steps yet doctor.

I have started my preps for step1 with kap vids.

done half of biochem.
started goljan path I hope I finish it in 3months. Not worried about path cause I have done cvs rsp kidney connective tissues from harrison earlier which I think
may help me in this journey.

done cvs pharm section (pharm being my strongest point).

so I have started now and received my id and password from ecfmg I will beging application process in a day and two.

hope it is a journey I have started well....

i! i am a graduate of 1988 from Pakistan. ive been in the States for 18 yrs raising kids. Now my youngest is 12 and im embarking on the journey towards USMLE. I did start a couple of times before but never went past Physiology,allways giving up at the slightest distraction. I have the kaplan notes and videos.What i really want is advise as to how i should go about it. Ive seen a lot of U tube videos on the subject but they are for fresh graduates. If there is any one out there in similar shoes please advise me. Or do u think that it is foolish of me to try after so many years?

Embarrassed feel powered and alive

Hi! after 2 days of desperately waiting for an answer, finally got one today. iI was in the dumps thinking no one cared about this old mom. i had started on physiology Kaplan notes and videos, going over FA at the same time to see what is important. But i know that if i do not do any questions related to the topic it may go away once i am on another subject. Some one advised me to get UWorld right now and do topic wise questions. What do u guys suggest?

I just saw some lectures by Dr najeeb on U tube. i liked them but they were too long. they dont have the full library on U tube,only few are present.
They are available for $99 .
Do i need to listen to them or is kaplan lectures enough?
As it has been a long time since i graduated, advise me about Dr Najeeb lectures.Will they benefit me?

For me it is more about this journey,, we gotta enjoy the ride, who knows what is in the fate, so enjoy and be happy right now, at this moment! Because all the stress will transfer on the family, and you will not be able to handle their stress, it would be really tough then, anyways, I hope you will make a nice time table for yourself first, morning routine, when will you cook/clean, evenings for family 100% etc etc, I mean start with long term plan in your mind, to avoid burnout! As burnout will be their

Anyways,
My suggestion is this,
Get lates kaplan books and videos plus latest FA, give a quick read to kaplan chapter,watch video then read quickly again, solve the qs at the end of each kln chapter, and superficially look the same thing in FA, finish this first read in 3-4 months, dont google or panic if you dont understand anything, kln have repeatations, just make a note of the areas where you cant understand,mover on,see if you can understand the second paragraph or not, if you can ,just mark that place,if not google that thing, read it on wiki.

Do uw after your first or second read.

After first read, spend 4-5 months, detailed understanding,may be with study partner.plus do uw.

Then focus on uw and memorize FA.

hi there! i really got into pharma, husband was gone for a week so got a lot of time. BUT i got side tracked and started listening to DR Najeeb lectures, they are good but very extensive and after 15 days ive just completed three chapters of pharma. please someone give me assurance that kaplan + videos is enough, i"m in too much of a dileema. Need advice ASAP.

Well! Let me just tell u about pharmacology as those are the ones I did( Dr Najeeb). For may be half hour of Kaplan lectures about the receptors Dr Najeeb has may be 6 lectures all an hour long. He is an awesome teacher, the lectures are really good if u are doing it for the first time , like in med school. I want to listen to them but if I do there is no way I will finish any subject in a month. Even a month may seem to most of u a bit too much for one subject but for me with kids and summer there are really some days when I hardly get any time. So! I have decided that for now on I will just stick to Kaplan videos, only using Dr Najeeb for stuff I don't get.

Hello every one, nice to see all of you here...your presence increase the blood circulation in my brain, and I feel young

Dr.Najeeb is good, but really really long lectures, if you can understand Kaplan, then do not go here and there, if not, wiki is good and quick source, even if wiki/google is not enough then keep Dr.Najeeb as a last resort!

Anyways,
I am doing things really slow, half a chapter a day, today I did half of hematology,but I guess these ups and downs will be there, I am trying to sleep early and wake up early...lets see!!

Thanks Dr Saaz I really appreciate your input . I just listened to Kaplan lectures today. I was on the ANS , they are not that bad, I just have to concentrate more.

Hi xyzee and all

Just woke up, since yesterday my brain is going crazy, what an experience, what a roller costar ride, couldn't sleep after exam, slept today after 8:00a.m

Well,
About my prep: You guys pretty much know everything, I did watch a lot of videos, even till last day, when all words jumbled up in last days, I put on videos, watching Dr.Fischer or Dr Fasalis helped me calm down,
Then MTB 2 I did multiple times, as everyone else says, medicine is good enough, remaining things are good in mtb 3,which I couldn't revise in last days.
UW is really really imp, do that many times.

About exam:
It was 50-60% doable, like you read qs and you just click answer automatically without thinking, than 20-25% qs were like two options looked the same, so that needed little bit of thinking, then these other 20-25% cases came from Mars, gave me all sorts of palpitations, stress!

In last 14 days, do your MTB 2 for medicine and MTB 3 for other subjects very good! Off course you must do uw! Focus more on your weak areas, rather, just repeating same thing again and again!

But when you study compare in your mind or with your sp, how would you differentiate many signs and symptoms of similar diseases, like Dr.Fisher say, what is different what unique

I will say more about this exam after my result, I don't know what will happen, let's see!

Wow what a journey..... You are really a fighter
I dont think that you have to worry of What ever your score you will get but going through this thread from beginning then i realize it started last year and ends few days ago ..... Im pretty sure you are a wonderful person inside and outside ... Expressing all your emotions , giving advices and struggling with kids and house and studying .... All the best in your future which im sure will be bright inshaa Allah . Wishing you good luck and remmembering you in my duaa in these holly days of ramadan . Keep up the good work
Oops about me , im an old IMG too and My final step (ck) will be july 18 insh allah .

Ive been completely out for about a month with family obligations. Its so easy to get side tracked, getting back on is hard.
I just read the forum today, Im so happy for u Dr Saaz, reading about your exam really motivated me again. I have to make the best of my time. InshaAllah

Best of Luck to both of you! I can empathize with your situation even more because my mom too had to give up her US residency dreams to care for me and my siblings. She has had second thoughts since then abt getting back into the game, once we had grown up, but she never quite gathered the required strength. Now she has all her hopes pinned on me.
I think that if you work along the correct lines there is no reason why an old graduate or specifically an old mom can't ace these tests. It's just a mental barrier that you have to overcome. The biggest pitfall you have to avoid is not to start underestimating yourself just because other people are doing so. Have faith and you will succeed IA!

Thank God I passed, 211, not a great score,but for me it is above 280!
ecfmg certified finally ,first attempt all steps!!!

I do not have kids to take care of and I am a fairly recent graduate, but I know how you feel, bc I work full time through my studies.

I looked through the old posts in this branch and did not find the answer to my question,
Do you mind sharing your UW scores, like first time and later on?

I am trying to make a sort of a range in which scores fall according to UW.
Would be interesting to know where does 211 falls into
