Lungs Low tidal volume in ARDS

Hi all:

Can someone please explain why in ARDS patients, we need to give low tidal-volume mechanical ventilation?

To reduce the risk of Barotrauma
Standard therapy is low TV with PEEP

yes, TV should be 6!

minute ventillation = TV * RR

decrease TV and increase RR to maintain the MV
[6 ml\kg instead of 10-12 ml\kg]

this is due to results of ARDS Network trial of 2004 where low tidal volumes showed mortality benefit
