Heart Indications of heart valve replacement

please tell me indications for valve replaement in MR AS and AR...

for MS baloon valvoplasty is DONE

if it fails... or cant be done - REPLACEMENT


for All regurgitant lesions....Before heart dilates too much n ef goes down...we must replace !!

Correct Answer

and yes , for AS

replacement if

1.SAD-syncope-angina-dyspnea ... i.e sypmtomatic
2.asympto bt severe AS with LVH>15mm or Abnormal response(symptoms) with exercise
3.When undergoing procedure for CABG or other cardiac surgery...(ek teer se do shikar...sort of )

dhruv,thats wrong i think. because symptomatic MS and MVP may not initially need intervention.

from intervention i mean surgery. symptomatic MS initially [more valve area with no pulmonary a. hypertension ]gets diuretics and MVP gets BB.
-for Symptomatic MS, if the valve area is more than 1.5 cm2 and pulmonary artery pressure less than 60, keep treating with medication .

i cannot find out the source online though

For symptomatic and assymptomatic patient surgery for valvular change is indicated when pt present with
For AR : Ejection fraction of <55% LVESD > 55mm
For MR : " " of <60% Left ventricular end systolic diameter of >45mm.

For AS treatment of choice is Valve replacement.Although diuresis are the best initial treatment but it does not have long term prognosis.

For MS treatment of choice is balloon valvuloplasty even in pregnancy. diuretics are the best initial therapy but it does not alter progression.
