Default High Anion Gap Normal Electrolytes - Reported Nor or Abnor?

This is a follow on to a previous thread, where a user said the scenario is unlikely, however I've chased this down and it is all possible and happens! Patient values Sodium 141, Potassium 4.38, Bicarbonate 22 and Chloride 99. All normal lab range.

What is the Anion Gap?
Would this be reported as a problem?
Is it significant?
What would your course of action be?
