Drug Colonoscopy and Infective Endocarditis Prophylaxis

A 64-year-old man is seen by the physician for his annual health maintenance examination. He has been well since his mitral valve replacement 2 years ago. At present he has no complaints. His family history is notable for paternal colon cancer at the age of 75 years. A fecal occult blood test is performed and is positive for blood. A colonoscopy is scheduled. What is the most appropriate pharmacotherapy to decrease this patient's risk for infective endocarditis (IE) associated with this procedure?

A. Amoxicillin
B. Ampicillin and gentamicin
C. Clindamycin
D. Gentamicin
E. Vancomycin and gentamicin
F. None

No prophylaxis is needed. I think, the only procedure that require IE prophylaxis is h/o IE, prosthetic heart valve (mitral only), structural heart disease, congenital heart disease, dental procedure in high risk persons such as those with above conditions.

Default

for prophylaxis of IE, you need to have risk factor plus chances for having bacteremia together. Colonoscope is not risk factor for IE, so you dont need prophylaxis.

Please correct me if i am wrong. : )
