Thyroid Hyperthyroidism and Radioactive Iodine and Pregnancy?

uw says radioactive iodine therapy is used in hyperthyroidism in adult and pregnency.
Kaplan says radioactive iodine is contraindicated in pregnency.

so what is treatment of choice for hyperthyroidism in pregnency and also when it is used in nonpregnent patients( Indication for radioactive iodine)?

In pregnancy u can do thyroidectomy.

Ya, agree. You cannot give radio-iodine during pregnancy. Surgery is better. For hypothyroidism, thyroxine can be given though. Thyroxine is not teratogenic.

yes thyroidectomy in 2nd trimester i think..how we have to manage in hyperthyroidism in 1st trimester of pregnancy??...

If patient is pregnant and thyrotoxic, then we have to weight the benefits of therapy versus the side effects. As all the treatment options can have potentials side effects for both the fetus and the mother. So, treatment is only indicated for moderate to severe thyrotoxicosis.

PTU can be used initially, as it is comparatively safer in the first trimester. Beta blockers can also be used for control the symptoms of tachycardia & palpitations. Surgery is safest in the 2nd trimester. A short course of iodides can also be used before surgery to Dec thyroxine levels acutely before the surgery. Radioactive iodine is absolutely contraindicated.
