Hello, I am wondering what are the step by step things we should do in nephrolithiasis? in my country it's urinalysis and US... but here in CK i see that besides urinalysis they mention AXR and US but they also include that noncontrast CT is the image modality of choice and US is prefered in pregnant women... I'm kinda lost in this thing, i dont even know if we start the hydration + pain control FIRST and THEN urinalysis and THEN imaging...
someone pleasE?

First step in Acute episode of Nephrolithiasis is PAIN management. Give Ketorolac or any NSAID.

For the second step I'm confused as well and I was about to post a similar Thread when I saw yours :D
I think its between a X-ray KUB (not AXR..im assuming A=Abdominal) or a USG. Most renal stones are calcium are so they should be visible on X-ray. But USG can detect any stone per se.

Ct scan is the 'most accurate diagnostic' test. It is done when you suspect a renal stone clinically but your 'best initial tests' namely Xray KUB or USG is equivocal.

Hope this helps :)
