When the patient does not improve with a course of antibiotics OR S/S and exam finding shows deterioration suggesting diverticular abscess or localized peritoneal signs indicating localized perforation.

it's the best confirmatory test & safer than barium enema in acute diverticulitis.

We should also avoid invasive investigation in acute condition as chances of perforation is high
