Pancreas Jaundice and Weight Loss!

A 78-year-old man is evaluated in the office because of a 6-week history of abdominal pain, diarrhea, and weight loss of 11 kg (25 lb). Physical examination reveals evidence of jaundice and weight loss. Abdominal examination is normal except for a palpable gallbladder. A stool specimen is negative for occult blood.

Laboratory Studies:
Total bilirubin =3.4 mg/dL
Aspartate aminotransferase =18 U/L
Alanine aminotransferase= 28 U/L
Alkaline phosphatase =110 U/L

Abdominal ultrasonography shows a dilated common bile duct and distended gallbladder but no stones. The pancreas cannot be visualized.

Which of the following is the most likely diagnosis?
A Choledocholithiasis
B Cholangiocarcinoma
C Pancreatic adenocarcinoma
D Primary sclerosing cholangitis
