Skin Dysplastic Nevia Management; Monitoring?

Quote:
Dysplastic nevi are melanocytic nevi exhibiting characteristics not seen in standard nevomelanocytic nevi. Clinical characteristics include size greater than 6mm, irregular borders and irregular pigmentation. Histologically, such lesions demonstrate varying degrees of architectural disorder and cytologic atypia. These nevi are usually monitored clinically and do not occur in previous wounds.
Friends, is this info correct? are dysplastic nevi monitored clinically? I was under the impression that we should punch biopsy it!

-the dysplastic nevi sp famlial one has highly ass with melegnant malenoma, but the melenoma rarrly result as transformation of one of existig nevi (i.e devlope in normal skin),so removal of dysplastic nevi dosn't prevent melegnant melenoma.but here the indication of imeddiate excitsion&histology :
Sudden enlargement in size
Development of irregular or notched borders
Inflammation
Increase in pigmentation
Mottling of previously uniform pigment
Bleeding and/or ulceration
Symptoms of pain or pruritus.
