Drug syphilis mom is allergic to penicillin

Hi all:

Kaplan says that even syphilis mother is allergic to penicillin, we should still use penicillin? but can we do this like in infectious disease, if patients are aleergic to penillin, with rash then we change to cephalosporin, with anaphylaxis then we change to macrolide?

Thank you!

to best of my knowledge we use penicillin and only penicillin [ even if it means desensitization] in tertiary syphillis...
in primary, secondary and latent syphillis we can switch to doxycycline. but in pregnancy we cannot use doxy so we use desensitization.
make sense?
tip
in syphilis q if patient is pregnant, close eyes and ans penicilin.

uptodate...

Parenteral penicillin G is the only therapy with documented efficacy during pregnancy. If penicillin allergy, skin test and desensitize as necessary.
