Medicolegal and Ethics HIV Positive Husband, Wife asking you about his condition

34-year-old male presents with symptoms of increasing fatigue, forgetfulness for the past year. There is history of IV drug abuse which patient stopped 3 months back. Workup of dementia is carried out. Patient is found to be HIV +ve. When the patient is informed of his condition and the prognosis, he tearfully asks that no one in his family be told the news yet. His wife, who is also known to the physician, calls the next day to inquire about the report . She says she is deeply concerned because her husband refuses to divulge any information. What is the most appropriate response on the behalf of the physician?

A. Ask the wife to come to the office so that the matter can be better explained
B. Call the patient and tell him the diagnosis will be revealed to his wife if he does not tell her himself
C. Inform the wife that the pathology report is not yet complete
D. Tell the wife to figure it out on her own
E. Tell the wife it would not be appropriate to discuss the matter at this moment

There is a similar question in the USMLE WORLD like this one and the answer was it would be inappropriate to discuss the matter at this moment ( answer E) i believe it was a very similar presentation and the other options are not appropriate

ok this question covers 2 things

1- CONFIDENTIALITY
you cannot under ANY circumstances tell the wife the husbands diagnosis as long as he doesnt ask you to tell her.



2- HIV reporting to PARTNERS

Its not your job.... u are just supposed to inform the authorities. It is they who get in touch with the partner.. and without disclosure of the details of the patient, SUGGEST getting tested for HIV.


so cos of these 2 things... u wudnt tell the wife anything.
