Neuro Meningitis Question

the question is attached below

the answer choices
1- CBC with DIFF
2- CT of the head
3- CSF gram stain
4- CSF WBC count
5- MRI of the brain
6- CSF culture
7- Coagulation profile

the best initial test is a CSF WBC count... which i know

but in this case
due to the age/college admission of the patient wudnt we suspect either St. pneumo or N. meningitidis and hence prefer doing a CSF gram stain instead?
(answer i picked)




the answer they gave was CSF WBC COUNT-- to diffn between viral n bact first
then a CSF gram stain
then CSF culture (most accurate)

The question asks for the the best initial test, so WBC is the most important of all them.

Well CSF culture = gram stain is both the most accurate test.

we could look also at glucose first to know (to treat or not to treat)
cuz if it's nl --> definitively viral
not nl --> bacterial, mycobac, protozo ..

but that's the case if they gave you all things in LP analysis
