Q. By what mechanism diuretics increases heart rate and worsens the symptoms of hypertrophic cardiomyopathy?

HOCM worsens in states of decreased intravascular volume due to increase in outflow tract obstruction. Similar to MVP.
The way it works is, increased volume of blood results in increased volume of the left ventricle and this pushes the interventricular septum away from the outflow tract and makes it easier for blood to exit the LV.
And as you know diuretics decrease intravascular volume.
