Default Q.why spironolactone is not given in HCM?

Q.Why spironolactone is not given in Hypertrophic cardiomyopathy(HCM) while other diuretics can help?

avoid diuretics in general. heart is dependent on preload .

I know, there is no benefit of using diuretics in HOCM (b/c it decreases preload and consequently worsens sx's of HOCM), but diuretics can help in HCM as mentioned in MTB (u can have a look over HCM>treatment section).But I wonder why especailly we avoid spironolactone???

Its also mentioned to avoid digoxin. So my guess would be because they cause hyperkalemia.

You can use diuretics in HCM, but not HOCM.

I agree with datson, spironolactone causes hyperkalemia which may lead to arrhythmias.
