Medicolegal and Ethics Mandatory Reporting of Medical Errors!

is every medical error reported ?

so , do we need to report EVERY medical error ? even if there was no harm

The question is not that of harm but of ethics ... Every error no matter how small should be reported regardless of the harm and the physician bears responsibility regardless of who commits the error
