Default 69-yr old man with RHD

A 69-year-old man with rheumatic heart disease
presents to the emergency department
complaining of a fever and weakness on his left
side. On physical examination the patient is
weak in his left upper extremity and he draws
only the right half of a clock. Shortly after his
presentation, the patient dies, and an autopsy
is performed. A gross view of the patient’s heart
is shown in the image. Which of the following
is a risk factor for the type of lesion pictured?

(A) Coronary artery disease
(B) Hypertension
(C) Mitral valve prolapse
(D) Prolonged bedrest
(E) Prosthetic valve replacement

I see vegetations? so probably MVP, an already diseased MVP is prone to infectious endocarditis and mycotic emboli towards the brain which would explain the fact he developed hemineglect, prob to the right parietal lobe.
