withdrawal bleeding is (as the name suggests) due to withdrawal of progesterone just like as happens in normal menstruation.

breakthrough bleeding
is due to too little estrogen as in OC Pills compared to normal physiological levels of estrogen. So, the endometrium becomes too thin (atrophic) but OC pills have enough estrogen to inhibit gonadotrophins and hence ovulation.

Breakthrough bleeding is mid-cycle bleeding. It occurs in case of low pill, POP. it is due to low level of estrogen. Endometrium atrophied and sheds. It auto disappears after 2 to 3 cycles mostly.
