The drug of choice is Trazodone, you can also give Mirtazapine.

Tx of Depression + Neuropathy use: Desvenlafaxine

Tx of Depressiobn+ Sexual incontinence or history of drug depende/abuse Use: Bupropion

Source: MTB and a shitload of wrong questions on these subjects HAHA

Depends on the severity of the depression/insomnia as well as age/co-morbid conditions since there is no single DOC (always remember now-a-days the drug therapy depends on the age/co-morbid conditions and other drugs interactions)

Elderly thin patient ( low BMI, loss of appetite) with BPH/risk of delirium >>>>>Mirtazapine since it has the least anticholinergic effect and improves appetite & wt.

Younger obese patient without risk of seizure/BPH and have chronic urticaria>>>>>doxepine since you don't want to increase his appetite/wt

Patient with severe insomnia doesn't want sexual side effects from Trazadone>>>>>>nefazodone
