Default listeria meningitis

how do you treat it? for sure you need ampicillin
do you give vanco+ceftriaxon+ampi OR just ampi?

Depends on what they give you..

For example if they specifically say you have L.monocytogenes then you only give ampicillin (got a question wrong in kaplan qbank because i put: Ceftriaxone, Vancomycin and ampicillin).

However that scenario is extremely rare, in Uworld they will give you an old patient with some comorbidities (by old I mean greater than 65 years old) and by comorbidities either chronic alcoholism or any immunosuppressive state like diabetes or something like that and that case you will have to pick the Vanco+Ceftriaxone+ Ampicillin combo

And if it says that the culture is gram positive rod (they don't say the name listeria) - do you stop the vanco and the cegtriaxon and continuing with only ampi?

Yup, you stop the other drugs and specifically target the organism, because if the other 2 didnt grow on the culture then thats it.

Dunno for sure what im telling tho, im speaking from experience, from what I could gather from qbanks and from what I have been reading so if you have another source stating the opposite then be sure to share
