Default patient with chronic cough and u think GERD

chronic cough + suspected GERD...what to do next ? scope or 24hr pH monitor and why ?

Scope? Because it is quicker than 24 hour pH monitoring and gives you rapid diagnosis compared to the 24 hour NG tube you will place on that poor guy and cause him discomfort

do ph monitoring that is the standard test to diagnose GERD , scope is an invasive procedure done if the patient is not responding to the treatment or has a bleed or malena ......... other wise simple non invasive test done .

Neither, just give PPIs (in the absence of alarm symptoms) which is both therapeutic and diagnostic.

24hr pH monitoring is only done if the patient does not respond to PPIs.

Endoscopy is only done if patient has/develops alarm symptoms.

Motility studies are only done as a per-requisite to surgical intervention ( Fundoplication)
