A 23 year old women was seen today she complained that her neighbor was talking about her, According to the neighbor , her behavior started 3 weeks ago after she was involved in a car accident. Since then she has been following the neighbors for several days and harassing them at work.She believes that the neighbors ate putting poison in her food and want to kill her.When asked why , she is unable to five clear explanation but insists that what she is saying is trues.She states that the voices in her head told her it is true and that your should stop asking question. While in the waiting room.you observe her to be dressed bizarrely and laughing inappropriately. Which od the following is most indicated in the managment of this patient?

A. Haloperidol
B. Clozapine
C. Lorazepam
D. Resperidone
E. Fluphenazine

Answer D. Reperidone.


This patient clearly has psychotic symptoms; therefore you would want to give her medication with the least amount of side effects. Choices A and E are typical anti-psychotics with many side effects. Choices B and D are atypical anti psychotics ; however clozapine is not used first line in the treatment of psychotic symptoms .Lorazepam is not an antipsychotic mediaiton.however it can be used in psychotic patients to reduce agitation.
