Default Cat Bite : NBME 2 Concept

A girl is brought 30 minutes after
being bitten on the forearm by her cat. Examination shows a small
puncture wound covered with dried blood. She is at increased risk of
infection for which of the following reasons?

A) Arm wounds are more susceptible to infection

B) Salivary enzymes in cats augment infection

C) It is difficult to cleanse pathogens from this wound

D) The Staphylococcus species involved is particularly virulent

E) Young children are less able to resist infection than adults

I choose B but was a random guess dont know whats the rational

Puncture wound are difficult to clean and debride specially the deep ones so more likely to get infection and admission is also indicated

for IV Ab

also the same thing is done to hand bite cuz structures have relstively low blood supplky and more to prone to infetion

Bites of the hand generally have a high risk for infection because of the relatively poor blood supply of many structures in the hand and anatomic considerations that make adequate cleansing of the wound difficult. In general, the better the vascular supply and the easier the wound is to clean (ie, laceration vs puncture), the lower the risk of infection.

Dogs STAPH
CATS PASTURELLA
RODENTS streptobacillus

Mx
Irrigate Debride Ab only if PUNCTURE BITE,HAND BITE, LATE CARE, MASSIVE CRUSH INJURY AND POOR GENERAL HEALTH AMOXICILLINBACTAM IF ALLERGIC CIPRO + CLINDMYCIN if MRSA GIVE TMP CLINDAMYCIN DOXYCYCLIN

ACYCLOVIR FOR MONKEY BITE

ADMIT AND CULTURE IFfever, sepsis, spreading cellulitis, severe edema, crush injury, or loss of function is prese
