Arrow 43 year-old with SCD

A 43 year-old man with sickle cell disease is admitted with an acute pain crisis. His only medication is folic acid.His Hct on admission is 34%. On the third hospital day, the Hct drop TO 22%. What is the best initial test?
A- Reticulocyte count
B- Preipheral smear
C- Folate level
D- Parvovirus B-19 IgM level
E- Parvovirus B-19 PCR for DNA

I think you are right.Answer is reticulocyte count.

In already diagnosed case of sickle cell disease,we will do reticulocyte count to find out ,is it aplastic crisis.

I was confused as MTB says,best initial test for sickle cell anemia is peripheral blood smear.It is to diagnose sickle cell anemia.

its reticulocyte count..it will help differentiate between apastic crisis and sequestration crisis........

ya in aplastic crisis= No reticulocytes
Sequestration crisis ass with increase or normal reticulocyte count

reti count to differentiate between aplastic crisis(no reti) and sequestration crisis...

B peripheral smear.
on the second thought u guys are right. we already know he has sickle cell. so we need to find the etiology of the crisis. so the correct asnswer would be A reticulocyte count
