Lungs How to differentiate the causes of pulmonary hypertension?

Hi everyone, a complex question:

How do you differentiate the causes of pulmonary hypertension? From what I understand, it can be caused by a lot of things but 3 of the main are

Pulmonary
Cardiac
Hepatic

I know lung diseases themselves can give pulmonary hypertension. Which then can progress to affect the RH and then the liver - causing cirrhosis.

Conversely, pulmonary congestion can be an effect of cirrhosis in its late stages.

In each of the above 2, there will be signs and symptoms of both lung disease as well as liver disease. Ascites. Jugular Venous Distension. In fact, some questions on UW have patients who are both smokers as well as alcoholic present with SOB and Pulm Htn and Ascites.

How do you then tell the difference of which is cause and which is effect? Is it a cirrhotic liver causing pulmonary hypertension or lung disease causing a cirrhotic liver?

Finally, the last question I have is how do you tell the difference between pulm hypertension caused by lung disease versus heart disease? Is it the PCWP?
