Bones Vertebral Osteomyelitis; Best next step?

Clinical vignette of an acute osteomyelitis of vertebra. what is next step in management?
a) x ray
b) MRI
c) bone scan
d) physiotherapy

I think x ray showing sequestrum/involucrum to back up the clinical presentation will be proper

I guess it would be : MRI of the bone.....after starting empiric antibiotics.....to visualize the extent of soft tissue involvement....anybody else??

well,i think... it should be MRI because bone damage in x-ray will appear after a several weeks of onset of osteomylitis. case isn't clear... if this patient comes after several weeks then x-ray should be the next step.....otherwise it's MRI.

Correct Answer b

for acute osteomyelitis of vertebra next step is MRI.......BUT for Chronic osteomyelitis its XRAY..(uw 2010)
but in MTB 2 next step in management is XRAY...

i think MRI ( kaplan surgery)
acute osteomyelitis: do mri as xray will not be showing any changes so fast.

i think it's x-ray the right one.......
plain x-ray is the initial step..although mri is sensitive(coz of no radiation) ,it can be confused with different conditions like fractures and tumors...

x ray is the best initial step in the absence of impending cord compression.

Bone scan if X-ray inconclusive.

MRI in the presence of impending cord compression.
