Bones Best initial test for Osteomyelitis?

MTB 2 says :

Best initial test for osteomyelitis is X-ray.

Most accurate is bone biopsy.

UW says question ID 4120:

Best test for vertebral osteomyelitis is MRI and do the blood culture as it allows directed antibiotic therapy.

This question has been discussed over and over again.

It's a really simple concept:

Impending cord compression (as in vertebral osteomyelitis) you go straight for an MRI since the consequences for the patient are devestating.
In simple osteomyelitis, do an x-ray which also serves as a baseline. If an x-ray is equivocal then do an MRI/Bone scan.

Quote:
This is not your typical garden variety osteomyelitis......this is vertebral osteomyelitis in which early and accurate diagnosis is a priority. This patient is at risk of developing spinal cord compression so an MRI is the most appropriate step.
