Bacteria History of TB with PPD not done!

If patient has previous history of tuberculosis with the x-ray showing scar now come to you with cough with sputum. He never had PPD in the past. What we will do now?

PPD testing.

Sputum for culture.

sputum stain and culture is the best step after xray in a symptomatic patient.
PPD is not useful in those are symptomatic or have an abnormal CXR. MTB 2.

There is no meaning and no value for doing PPD if there is previous history of TB cuz the result will be +ve, and also may be negative if the patient is immunocompromise, so PPD will not change your plan for sputum culture if you suspect TB in differential diagnosis.
That is what i think
