Random :

If epinephrine is markedly elevated, where is the pheochromocytoma location?sympathetic chain?adrenal med?

Answer : adrenal medulla or organ of zuckerkandl, because non adrenal organs lack ability to methylate norepinephrine to epinephrine.

i think you're almost near to your goal....stress on your weak points in nbme..
i dont think u should post pone with 1 month in hand............there are many experiences in this forum ..read them in your freetime....they improve like 20-30 marks in last month itself and also dont rely on single test.....test another in coming 10 days to decide.......i'm also in the same phase(i got only 207 in nbme 4 given last week)...you have many chances to improve buddy...keep on concentrate........dont lose hope...goodluck

I dont think I can take the exam by may since I have my CS scheduled in mid may, but still I would like to join the thread for inspiration. I'm doing 2 exams almost together, wonder why but its quite overwhelming I am doing respiration q's from UW. Just did 25 q's today, read MTB & did 3 cases for CS exam. Wish u all the best guys.

Default deciding exam date

Okay guys, I am currently completely messed up prepping for CS and CK. CS is may 13th. Will be taking CK end of may. So How do i decide my exam date for CK and book a slot. Have done MTBs once, UW 20% left ( 71%) . I don't know if I'm really ready but I need to finish taking exam by may end as I start obsies in june and doubt if I would be able to manage both

goodluck for cs......looks like your uw percentage is good....so give an online nbme 3/4 and decide yourself----though many say they are outdated,you can roughly estimate yourself that you are near or far from your dream score
i'm also almost in the same position....with my ck on may 9 and cs on june 26...but still finding my way not to postpone my ck........
goodluck...post your updates too

Default Count me in!

venky2600: If you dont do 60 - 70q everyday, uworld just doesnt finish. and some of the good scorers had that routine. I barely managed half a block in the beginning and then shifted to one. Now I have loads of uworld still left and all other sources completely neglected. Try and do 60 - 70 questions with quality study.

Guys I am on Uworld. 40% is still left because I have been very slow. Ive covered all subjects except IM. On that these days.

thanks @brainiac ---ya buddy i already did finish uworld a month ago. i asked it coz in revision mode i'm doing like hardly 80 q's daily with a 20-30q's of kaplan qbank,but i just wanted to ensure that i'm not at the level of minimum number of questions doing per day..now i got the answer from you and thanks for that..
yup sometimes it happens that we cant able to do half a block per day....dont worry buddy.... you can finish it now,just concentrate more on the block explanations so that you can able to answer from that topic even if it's given in anyway(just be sure that you may not be able to get time to revise it again,so prepare it well).......and when is your exam?....if it's in next month then try to do 2 blocks per day at any cost(coz many say exam is 50% minimum from uworld)...so divide it according to your schedule.......keep updating buddy...goodluck

@rupesh-----thanks -yup that's gud you're relying on uworld alone.....i finished my uworld month ago so, i started doing kaplan qbank 1 block /day...mainly for time management along with u world offline.......now i stopped it ,only doing uworld revision now(coz i'm forgetting many imp. topics))....goodluck buddy

Thats awesome! you're already done with one read. and I guess the max speed in the second read could be 3blocks per day. If you achieve that it would be great. Yes I am trying to manage atleast 2blocks a day now. because I have to revise this too and I dont have the luxury of time at all. I am trying to do MTBs alongwith uworld.

ya that's best idea you're doing mtb's with uworld.......you can complete both in time by doing both along(i also did the same thing)....yup 2nd time(but my nbme score is not appealing ,so started heavy revision of uworld) ,but i feel like i'm unfamiliar with some questions and few questions are getting wrong too...so taking time to read explanations again(anyway it's good though).....yup it'll take time to do 2 blocks a day with explanation(first time), but be advised to sit for long time study during last month ,coz for success hardwork is must right..?
goodluck

I did the same question yesterday and got it wrong But read it again. Says patient is unresponsive and CYANOTIC. So intubate first and then give the drug. Hope i'm right If no cyanosis then it would be to follow the usual drug protocol

dawns effect>>>>increase Growth hormone>>>midnight hypoglycemia>>>>>treatment by increasing night time insulin dose
somogyi effect>>>decrease in bl sugar may be due to increase in insulin dose>>>treatment is reduce dose of insulin at night.

as dr.fischer says Too much information going thru my brain and too much information driving me insane.......
i think enough for me for today..

Eh,im a little confuse..for dawn, u increase insulin = making even lower glucose → even more hypoglycemia? But isnt that is the dawn effect? Also which book u get this from? Cz i realize i hvnt come across it at mtb..

seeing at your step 1 score..i dont think you scored that bad as my nbme.....but anyhow i think you're just few points away from your target...just be focussed ,there's still time(end-may) to improve rather than wandering n worrying...
and what are uw tests?....as far as i know ther's only 1 uwsa.....

i'm afraid you wont get disheartened by my words.....goodluck

Thumbs Up

today's schedule----->morning-completed GIT(excluding liver) mtb2,kaplan(few),uworld
evening----endocrine mtb with uworld

today's high yield points---->from uworld

1) dementia+ myoclonus+ supranuclear ophthalmoplegia + diarrhea(malabsorption)+pigmentation ======whipple's --> PAS positive in small intestinal mucosal biopsy....treatment--ceftriaxone followed by TMP-SMX

2) elevated BUN with normal creatinine-----> upper GI bleed , steroid administration

3) elevated creatinine with normal BUN-----> due to drugs that decrease creatinine excretion--->ex--TMP-SMX,cimetidine etc....

thanks..keep motivating

yup you're right.....but here dawn effect is---> growth hormone releases after falling asleep.....decreases insulin(NPH) sensitivity which is given in the evening---->so as pt. has less insulin b/w 3-8 A.M--morning hyperglycemia is seen in this pt..

hope it clears.....actually thanks for asking....i got a chance to revise it..

i already told you ,they dont correlate with exam,as they are old.....just improvise with your time management from those exams
actually to say i also got 40 mistakes in nbme 1 which i did 20 days back.....just notice where you got wrong and prepare well at those weak topics dude....
if you want nbme offline correlation table given by nbme blogspot.....then it makes 219...but i dont believe it coz a week after it i gave nbme 4 online and you know my score in it.....
we should prepare like making max 6-7 mistakes per block....hey ! it's just my opinion dude.....

yup...it's difficult to manage time..but with 132 q's a day...i think you can easily manage that by the exam day..dont worry..
ya offline nbme's cant figure..but you can know the subject (i mean gynec/peds/medicine) which you got wrong and be well prepared for next nbme
dont worry....now you know by your performance....certainly you will improve by next nbme......

hey dude....it happens to all for 1st 5-6 blocks......just go through explanations thoroughly with wrong options too(read alongwise mtb or kaplan whatever similar to your old schedule)......i'm sure that you will be improved by time of 8-10 blocks. ...and dont rely on uworld percentages ..they arent assessments...use it like just another textbook ..to prepare.....assess by nbme's and uwsa......
goodluck with your prep..

Default I think your score will be biased if you take NBME 6 , so it is better to try 3

For infectious diarrhoea i THINK I learnt from UW lactoferrin is better than Stool WBC(Correct me if i am wrong)
i think usg will never be the sole question coz it only differentiates solid from cystic lesion(thyroid nodule>>>>tsh>>>>fnac)

yup regarding lactoferrin, i also read it in mtb,but just making sure with what option should we go ,if both are given...?
( i mean some explanations say that stool wbc is easier to perform and in reality too we will go with stool wbc right..?----just my opinion)
