Default How do you differentiate Otitis externa and Malignant otitis externa?

Can anyone tell me how to clinically differentiate otitis externa and malignant otitis externa? This is a usmle world spoiler so if you havent done this one in U wolrd , please dont read the answer.

Malignant otitis externa is a severe form of otitis externa which has a tendency for the immunocompromised and spreads into adjacent bone and soft tissues causing complications.
The case will classically describe an elderly diabetic (immunocompromised) with unrelenting ear pain that doesnt go even after treatment with antibiotics. or the case may hint involvement of the TMJ.

Cholesteatoma may be visible if they throw that in the question

Isnt cholesteatoma more related to the middle ear rather than the outer?
