Neuro Pituitary Adenoma and Dexamethasone Test


MTB 2 p.120

Why do we scan brain and do petrosal sinus sampling if we can exclude pituitary adenoma by being not suppressible with high dose dexamethasone test

maybe to confirm that the source is pitutary
and not from some ectopic source

inf. petrosam sampling is done in case of high ACTH, but equivocal result on MRI or chest CT

cant we just exclude pituitary source of ACTH if high dose dexamethasone test does not suppress cortisol output ?

So correct me if I'm wrong please.. But I think the sentence should be.. 'Once the ACTH is elevated and DOES suppress with dexamethasone, scan the brain with an MRI.
Suppression of ACTH by high does dexa = pituitary source, therefore we would need to do an MRI or sample the inferior petrosal sinus for ACTH.
