Default acute prostatitis diagnosis

while in MTB 2 it said that prostate massage increase the diagnostic yield of urine culture , in Uworld it says that this is avoided as it leads to bacterimea ?
can anyone explain

You avoid performing prostatic massage because it can lead to bacteremia and sepsis.

Most of the time the dx is clinical, you can get culture of urethra secretions if you want.

When MTB and Uworld are different.. Follow whatever Uworld says, they are the law/norm/bible regarding USMLE

In acute bacterial prostatitis urethral instrumentation is contraindicated but not sure of massage

Even kln surgery notes says that u shouldnt d prostatic massage...it can lead to septic shock..so yea its wrong in mtb i guess!!

totally agree , i think MTB is wrong , there also other subjects taht i noticed differences between the 2 ,
