Timed random is the way to go in my opinion because it will prepare you better for the actual day. Its tougher but better to prepare tough than face problems on the exam.

Default Start subject-wise and end in random

You better start with subject-wise and then end up doing randomly.

Once you feel that you have finalized reviewing certain subject then start adding it to the pool.

For example,
Finished revising internal medicine, starting doing internal medicine questions. Then when you finish reading the second subject let's say pediatrics, you start doing IM+Peds questions, then when you finish surgery you start doing IM+Peds+surgery questions. So forth until you reach a point where you are doing all subjects in a random mode. Of course if your approach is specialties rather than subjects then you can do that accordingly, for example cardiology then cardiology+neuro then cardio+neuro+infectious ...etc

If you already have finished reading all subjects/specialties then it's OK to start the qbank in random mode.

This issue has been discussed before here;
U WORLD random or subject wise ????
and here
Subject-Wise questions not random

Even though the above threads are about Step 1 prep but the concept is basically the same.
