hey guys/gals, how many chapters/pages are you studying in a day from KLN psych and are you doing qs after reading everyday or waiting to finish the subject then doing the qs ? Thanks.

I did mtbs quickly, then started kaplan questions, haven't revised kaplan notes or done notes for Ck. wanted to check what i remember from step1.
Before starting u world i will give one read to first aid of step1 biostats and psych.
By the way of doing u world i will read ck note for psych. And do couple pf biostats questions every day for warm up. I have done ten questions in the morning of step one exam. Really good thing for brain warm up.

I have not done kaplan explanations. i was taking just a quick look at incorrectly anwered ones. Now after first read of everything decided to move strictly with uworld. i hear so much about being it the best study source can't wait anymore.
One month should be more than enough for kaplan. Kaplan Q bank is definately good for time management.
Strict regime from tomorrow, throwing cell phone away.
More time goes, more forgetting previous stuff, so need to be quick and smart revising everything possible throughout the day with 1 or 2 blocks of uworld. October 18 maximum 3 months before exam

I love usmle world..
You'll love it twice more if you have done Kaplan as a first question bank

Although hate doing explanations and running from mtb2 to mtb3 to step1 biostats for annotations. But there is no other way.. 1,5 block of explanations so far..Wasted some time.. Hope to achieve 2 block speed. And start some reading along the way.. Thinking a lot to remember some stuff from obstetrics.. Need to revise it..

Hope you guys are not goofing around.. Keep up hard work.. Anytime you will start complaining about Ck remember that you have done step1 already..

Anyways about the kaplan book for psych, Its am finding it good!!
Can't compare with step1 kaplan, didnt go through it.
Lot of work still to do. Long way.......

Glomerulopathies
Biliary tract disorders: primary secondary cirrhosis, sclerosing cholangitis
Hereditary hyperbilirubinemias

Hyper, hypo thyroidism and parathyroidis.
Electrolyte disorders

Renal tubular acidosis

Number needed to treat- 1/ARR,
absolute risk reduction- event rate in placebo- event rate in treatment group,
In other words incidence on one treatment group minus incidence in other treatment group.
absolute risk percentage ( RR-1/RR)
Number needed to harm 1/ Attributable risk (subtract)
Relative Risk: divide

Spec and sev do not change with dx prevalence, fixed pretest stuff
PPV and NPV do change, post test stuff
Increased prevalence=increased PPV, decreased NPV

For dds ration and relative risk we dont want 1 to be included

If confidence interval crosses zero accept null hypothesis, i.e. Drug does not work

1sd- 68%, 2sd- 95%, 3sd- 99,7%

Dont know how but so far answering nearly all abstracts and biostats questions correctly. Am at >90 percentile in psychyatry good for me love the subject hope to nail longest weirdest questions on exam

Of course super high yield the double hunch diagram: which i can never remember what happens with moving cutoffs either side. Only thing thats stuck so far in my mind is that:
SPecificity goes together with PPV
SeNsitivity with NPV
I.e they change in same direction

vice versa: if specificity is increased NPV decreases, sensitivity increases PPV decreases.

Increased sensitivity means decreased false negatives
Increased specificity means decreased false positives

How are we still normal after all these...

3 tear old copies "circle", cross
4- quadrate
5- rectangle
6 -triangle
7- diamond

Its all about diagonals that need more precise work so triangle and diamond later..

I finishd Kaplan notes with Mtbs didn't kill myself about annotations and gave a really quick read to notes. I don't know what to advice.. In both mtbs you find all high yield condensed stuff from notes. But generally mtb is hard to read alone. You read it so quickly as if you know everything..

Uw blocks are getting harder. Hardly holding on 70% 7 blocks so far in 4 days..
Hypotension and sleep disturbances have settled in.. It means everything is going by plan..

Attributable risk -difference in risk bw exposed and unexposed groups and also used to calculate NNH=1/attributable risk
Absolute risk reduction-is risk with a treatment as a compared to placebo and used to calculate NNT=1/ARR
ALSO there is ATTRIBUTABLE RISK PERCENTAGE = RR-1/RR

Leave kaplan bank alone. And do uw twice. When i finish random, i plan to do subjectwise with mtbs and review everything in subjectwise order.

Bad news, i probably will not be able to do CS for match2015.
But ck needs to be done by january as planed im too bumped up now to wprry about anything.
Percents are better today second block 73%
Everything else step by step..

NBME, UWSA, Qbanks Averages, Correlation with Real Step 2 CK Exam Score
This thread is what I'm focusing on now..again and again. Need to finish UW random timed with >70!! And buy eligibility afterwards.

I would use mtb3 together with kaplan and mtb2. Try to consolidate info in one book. Easier in mtb3.
No more gaps and goofing around here good luck. Share your routine to be responsible fro what you do daily

Yes kaplan ped is complete resource, i also found surgery boring and still left with second part which is i hope more interactive question answer format.

Guys anybody knows how much it is in asian region for step 2 ck complete payment? Around 1000? Or 1100$? Or more? It does not show me untill i fill whole application. Need to make transfer. Just if anyone has recent info. Taking eligibility guys. December-jan-february. Or jan-feb-march

Its 23:00 tempted to do second block for today but afraid to spoil percents. Did explanation of one and half block today. Have not started reading though. I need to do some reading along with questions..

I don't know how much it is in Asia, but it shows it in the later half of the application process...dec-jan-feb I would say. Since your prep seems to be going fine, you should be ready by then.

Armenia is Asia right?
I think the same dec jan feb. I prefer to take december also. Maybe i will be ready before christmas who knows.. Will be finished with UW and thorough Mtb revision in november. Then NBME will show how to proceed after.

I am actually from Georgia. Beautiful small country Georgia, gate between europe and asia two cultures fused together.. Briefly we are european looking caucasians with Asian temper.. And what makes us all unique on this forum.. All preparing for usmle steps. Let's do it, like a boss..

Dear Friend of course I know where is Armenia, I've been there this summer, and liked there very much. Prometric centre is like heaven. quiet one. and I felt pretty much like at home. Hotel was very nice also 7 minutes from Prometric. Planing to visit this winter again. Nice thing that we are neighbours.

i had a dream yesterday.. that i took exam with preparation as I am now, moment of checking my score online.. was not happy with my score
Let's work fast and hard. don't waste time. and soon we will be free from this monster exams.

Did one block- 80% that felt so cool. Plan do explanations, And revise and re annotate and re-everything ob gyn, cause those are questions i just kill myself on while answering and never am completely sure.

Thats great!! You are going to be sooo ready to take the test in december. My husband's on call this weekend, so i might get some time to study. Weekends are really hard for me to study. Miss the days when i was a student and could study for hours. I dont know how you do it with kids.

What about kids they have grown up with my exams.. Especially the little one. I'm nearly avoiding kids during day as much as possible, although taking them to daycare and school etc.. The small one usually calls me: "this girl is crazy.." And weekends don't remind me.. She just seats on my table and books and enjoys the stay.
They will grow up and won't remember anything except that their mom is a good doctor by time they grow up just try best to find free time for study.. Otherwise you will be so irritated and you won't be good neither for kids nor study. Don't ask just demand!!! help from everyone to give you as much free time for study as possible.

P.s. I tried my best was reading like crazy but anyway it took me 4 hours to do a block with explanations.

I must say, you are so awesome!! Im sure your family will be really proud of you when you are done with everything..Well, i dont have so much going on..I dont have kids as yet..So, I have to pay attention to my husband alone.

Prometric center in armenia.. I am armenian but never been there.. J started to study USMLE when i came to USA.. I came to usa specially for to USmLE.. And fir that time i didnt know that there is prometric center in armenia...

Done with Pediatrics. Tomorrow is rest. Will start Gyne/Obs on Monday. It is my first reading and planning to take exam either end of December or Mid-January.Only covered first reading IM and Pediatrics. Keep motivated.

Finished surgery..Yay!! But not doing so well on usmleworld q bank...did 9 blocks so far, got 64% cumulative...Im running short of time, which never happened with step 1..

Had a moderately productive day...finished surgery and did a couple of blocks on usmleworld, not the explanations though...Have to start revising Peds tomorrow..Hope tomorrow will be good too..Good luck and good night guys!

No worries.. Its just beginning my cumulative is just 68% 13 blocks so far. Keep on working it's gonna improve if we work peristently.
Sunday morning, bad news the minister who kicked my out of job will be the prime minister soon
Good news: todays plan, making a good chart of primary and secondary amenorrhea managements on blank pages of mtb at the end of gynecology and read breast from kaplan.
Half explanations from second block i did yesterday (only correctly answered are left) and one new block.
Also any other subject reading from mtb would be highly appreciated from me but can not manage doing that so far.

Going to revise peds...its got the 2nd highest no of questions in usmleworld..and the kaplan videos were sooooooo boring, i dont remember anything from the videos...hope i can revise the initial chapters.

So sunday is over, Georgia has new president. And my private enemy turns out to become prime-minister soon

Did block on 84%

Trying to pull together mtbs for obstetrics at least. Sooooo hard. Hardly managed to do one complete amenorrhea page in mtb2.
So it is impossible to put mtb3 gynecology in mtb2 (8pages only ridiculous)
Bt will try to bring at least obstetrics. Its spread with important points through both books.
I really suffer on obstetrics questions. Maybe i will watch videos rockstar, cause this subject is real trouble, i never know exactly when to deliver, how to deliver...what to deliver

What about peds, i have heard someone say: exam felt like pediatric boards. So lets reinforce peds as much as possible.
Doing just fine in psychyatry so far, withour revision.
So started to move a little bit with Mtbs and some reading from kaplan.
I think it is more efficient to do one block thoroughly or maximum 1,5 block, and some good reading of specific weak for me chapters.

Trying to pull together mtbs for obstetrics at least. Sooooo hard. Hardly managed to do one complete amenorrhea page in mtb2.

What about peds, i have heard someone say: exam felt like pediatric boards. So lets reinforce peds as much as possible.
Doing just fine in psychyatry so far, withour revision.
So started to move a little bit with Mtbs and some reading from kaplan.
I think it is more efficient to do one block thoroughly or maximum 1,5 block, and some good reading of specific weak for me chapters.

After watching all the videos, i think Conrad fischer and Elmar sakala(OBGYN) are the only ones that are useful...I love this guy...like i said, he revises everything at the end of the topic...So go for it!!

I remember someone saying they got a lottt of questions from peds..Peds is like mini internal medicine...Trying to revise peds, lets see how it goes.

I did Kaplan videos only for obgyn and like it.. Just took long time to me to finish..
I am doing Emergencyedicine from kLN and mtb2.. Hope to finish today

Yeah quite a lot questions from peds, gyne, arrhythmias and cardiac emergency managements also kill me... So a lot hard work to do..

Good morning! 82% really don't know how i managed, block seemed way too hard. I would say ambiguous not hard.
So far only key to answering questions EXCLUSION, only thing that helps.
So doing some reading for warmup before doing block of questions. Wanted to review pap test workup but proceeded without doing it and missed 2 pap qs.
Rockstar quit that job i don't cook, i don't clean (except my study corner), i do take shower sometimes everyone in the house is so scared of me that nobody dares to ask for any "taking care of them proposals"

Have a good day and stick with study, turn off cell phones and all social networks. You won't miss anything from not being social, others will miss from not having you around

Default Bone Tumors differential

1. osteosarcoma- "sin burst"appearance, Codman's triangel - periostal elevation.
ALP LDH elevated. Metaphysis of long bones (maybe a/w familial retinoblastoma)
on PE tender soft tissue swelling !!! pain not relieved by NSAIDs. (first 2 pics)

2. Osteoid osteoma- benign, cortex of dyaphisis, <2cm with RADIOLUCENT core (osteoid), surrounded by rim of reactive bone. Pain quickly relieved by NSAIDs. (3rd pic)

3. Ewings- ostelolytic lesion with periosteal reacton, periosteum lays down bone to protect from fracture, "onion skin" appearance.
small blue cell malignant tumor, t11,22. aggressive but responsive to chemo, DIAPHYSIS of long bones, pelvis, scapula, ribs. (4, 5, 6-th pic)
difficult to diff radiologycally from osteosarcoma. they have also mentioned sunburst pattern for Ewings. so probably its best to stick with Diaphysis and typical sites for Ewing's and probable systemic symptoms more pronounced than with osteosarcoma. don't know how else to differentiate.

Default Pap Smear and CIN management

could not have been more beautiful indeed!

So
Start Pap from 21, despite onset of sexual activity.
21-29---> every 3 years
30-65---> every 3 years cytology, or every 5 years cytology +HPV testing.
>65---> no screening if recent prior tests negative.
No screening---> if hysterectomy for benign causes (i.s. no previous history of precancerous lesions, cervical cancer, or DES exposure)

ASCUS----> 21-24----> repeat cytology in 1 year
>25 -----> HPV testing---> if positive do colposcopy, if negative- repeat Pap and HPV testing in 3 years.

MTB sais do repeat Pap in 3-6 months and order HPV testing.
Had trouble with this in question, cause they gave both as different answer choices.

SO it is clear that if 2 consecutive Paps show ASCUS, or HPV16 and 18 are found or ABNORMAL Pap ------> colposcopy and biopsies.

Endocervical curettage---> all NONpregnant undergoing colposcopy for an abnormal Pap
Cone biopsy---> if Colposcopy and ECC are not consistent (i.e. abnormal cells were not biopsied) , abnormal ECC histology, endocervical lesion, biopsy showing microinvasive carcinoma of cervix.

Observation for ----> CIN1, and CIN2,3 after ablation or ecision
Ablative (cryo, laser, electrofolguration)or excisional procedures (LEEP, Cold-knife conization) ----> CIN2,3
Hysterectomy----> biopsy confirmed, Reccurent CIN2,3

Pap in pregnants managed same way except ENDOCERVICAL CURETTAGE.
CIN/Dysplasia- pap and colposcopy every 3 months during pregnancy, repeat Pap and colpo 6-8 weeks postpartum. If persistent lesion definite treatment.
Microinvasive cervical cancer---> cone biopsy to rule out frank invasion, deliver vaginally reevaluate and treat 2 months postpartum.
Invasive cancer <24weeks---> radical hysterectomy + radiation
>24weeks---> conservative management up to 32-33 weeks, cesarean and definite treatment.

Classification of Pap Smear:

ASCUS----> intermediate smear
HPV, Mild dysplasia, CIN1---> low grade squamous intraepithelial lesion LSIL
Moderate, Severe dysplasia, CIN2,3, CIS---> High grade HSIL

Cancer: Invasive cancer (management: cervical bipsy, metastatic workup----> hysterectomy. if mets, tumor>4cm, local reccurence, positive margins---> Hysterectomy+ adjuvant therapy radiation or chemo)

HPV vaccination:
Quadrivalent HPV recombinant vaccine (Gardasil)---->8-26years
Sexually active women can also receive vaccine but in above age range only
not recommended for pregnant, lactating, or immunosuppressed women, also recommended for men same age to prevent Genital warts. Vaccinated ones should still follow with Pap smears.

I have started GYN/OBS. From friends recommendation am starting from Gyne first to be followed by OBS. What do you think friends? Next 5 days are for these subjects.How wonderful it it also to listen kapalan lectures with 1.5 speed. I reduces the time by one third. Have a blessed day.

Do first gyn its more or less familiar. I am trying to revise them from MTBs. when you finish tell me if it's worth to watch Ob/gyn videos. although I know you are anytype of video guy

I wish i could do that...If i dont clean, the house remains dirty..My husband doesnt care..I like to keep my house extremely clean..But can leave the cooking part to my husband may be..

Of course Fb deactivation is number one thing to do, you can never start serious preparation without that. Cell phone is another big distractor too. as are husbands, boy/girlfriends and just friends, colleagues and every human being in this world when you prepare for USMLE.

Happy u r guyswho canwake upin morning do nothing at home dont care about cooking and clening andjust sit and study buy another world.be not married and have aportunity to sit and study only..... so be housewife and do USMLe its veryyy hard.... I have not been married when i tookstep 1 n could study up to 12 hours now... Max 4-5(((((

I know!! When i first started preparing in India, i got a lot of time to study..I didnt have to cook or clean...but ever since i came to the US, its soooo hard to study..have to cook, clean and do all the chores without any help...i know there are many women out there who do it, but i cannot give my 100% bcz of that...I definitely miss the days where i could relax and study for 12 hrs.

Dont forget you have more emotional support than singles.These are also the times when you really bring the best out of you. Hold on mama; we expect you to perform better not less from step 1. Had a fairly productive day...revised peds quite a bit..did a block in usmleworld and got 59%...and my cumulative percentage is 64% now (went up by 1% in between)...May be i need to take a break from the q bank and revise everything properly before doing the questions...good night guys!!

U r absolutely right! I am happy that i am married.. I was alone in US and far from my family and of course for to study had a lot of time but i felt lonely and board alone.. Now i know its hard but thanks God i have perfect husband who try to make my life easy and let me study whenever i need...

Default Panic mode activated

This is human rule we always wanna be there were we are not, and wanna be as we are not. When your single wanna be married, when you're married wanna be single. The main thing is that everything and everyone be supportive during this period. No matter spouse, friend or this forum thread, which helps us to support each other. I agree with Romerez, happily married ones have big emotional support, so girls try to manage your time as much as possible and everything will be fine. I guess you both are newly wed, don't worry it will get worse

I have 10 and 5 year old daughters, and i am a very clean person, but i have to do this test fast so cooking and cleaning can wait. and after my terrible step 1 experience with two very hard jobs with night shifts and 4-5 operations a day, oh yes i also envy myself that i stand up and don't care about anything except studying.

73%, my cumulative 70%. Buying eligibility today.

Rockstar don't worry, i had percents like 50 and 59, 61. Don't stop doing questions just do explanations very well and work on weak areas. I nearly finished review of ob gyn from Mtbs although again missed sequential questions about fetal demise today.
Doing questions keep us on check, with percents, so just reinforce the high yield chapters. And stuff that make you suffer while answering. Review review review s much as possible and exclusions exclusions exclusions for answer questions. When you are out of blue just try to come down to at least most probable 50/50.
Little social today with registration stuff and upcoming pediatric surgery licensing exam.
Good luck guys, work hard and you will feel so satisfied.

Had a lovely day yesterday, was able to finish my target. Continuing with obstetrics.....was scared of this subject before. But continuing with the spirit Come at me bro, show me what u got ....

I registered, DrAGA Armenia is Middle- east I found step1 financial transcript and changed Asia- to middle east region. they are the same actually free zone1 but i'm getting to nervous with details when submitting ecfmg applications.

Talked to ECFMG certified friend. He calmed me down a bit, he said: "exam is not way harder than UW as people say" , so god help us. Am already in total panic.

Missed working hours today do to this paper work. Gonna go and kill today's UW block explanations, finish Ob/GYn from MTB, Do HIV and dermatology from Kaplan notes. and work harder harder than ever. have to finish this soon hopefully with >230 God forgive me for being 8 years graduate

Thanks..Will keep that in mind..I was really good in Neurology and Psychiatry on step 1, but not doing so well on step 2..I guess i have to review everything once im done with peds.
