You're right.. but we are different from other females, not stupid girls and we gonna get nice scores
Tried my best to bring mtb3 in mtb2 and i guess these efforts won't bring me any good. Will read both books for other subjects.
Good night girls and boys.. So tired have tinling and numbness in my legs from prolonged sitting some people not appreciating our effort stupid people around never let them get you down.

We gonna be just fine, just keep patience and hard work..

You call fair performance >90 yeah? you gonna kill this exam, and i am sure when you finish first read of everything with q banks, just after one good review you gonna give nice Nbme and uwsa score and rush quickly to exam.

Good morning had a congested one hour 70%.
Didn't eat chocolate suffered through whole block i thought i would get 50% again, changed the last answered which i reluctantly reviewed from correct to incorrect one.

Good luck everyone, we are all keeping each other really motivated.. Will start second part of surgery from kaplan notes today after explanations

Good day guys!! Am disorganized today because of some social work. Also kind of sluggish did worst block of my life, i was so slow and 68%

Medicine doc you should read kaplan at least once. You Must do other subjects with kaplan. Generally it will help you with getting along with mtbs. Do kaplan then you narrow down to things you have to cram all the time.

Missed aspirin tox blood gass and blablabla need to read mtbs along during the day am forgetting things.

Okay i hope this is cause of dentist in the morning for >2 hours that im so out of shape. Gonna do explanations and continue hard work as usual!!

Wanna kinda have some rest, but no way ain't nobody got time for that.
My rest will be doing 1500qs for my home country licensing exam

Good luck guys work harrrrdddddd!!!!!

I dont know what's so monstrous about the exam...I mean, dont you feel good about knowing so many things that you didnt know before...I think we should feel of it as a journey and go with the flow...Its challenging, i agree, but what's not? This is how i see it..I may look lame to people, but, that's what i think

Continue with the motherland OBS. How wonderful it is; we all here passed the greatest challenge in life "second stage of labor". Nothing is more difficult than second stage of labor( doesn't hold true for dudes born with C/S). Have a blessed day.

that's what im saying all the time nothing is more difficult than second stage of labor.

Really bad day today.. I saw scales at dentist' office and could not resist temptation of weighing myself..
So i have 17 days and about 30 blocks left from these 17 days 14 days will be half day excessive training in laparoscopy

I definitely need to prolong prescription and i think that's okay to do uw in a month and 2 weeks is not it?? Why should i worry.. Or hurry.. Am i hurrying anywhere this thread says january ck takers so im on normal track..

Keep up working guys, lets enjoy this crazy journey
God help us

Love ipad app for Usmle, it made my life so comfortable.

Persistent bereavement blablabla disorder these dsm guys don't have better job i guess
Still missing congestive heart failure questions: starting to panic as long as i see buzzwords: sob, dyspnea, jvd, edema, crackles on lung base, blablabla, don' t have any clue when i see ecg, but still answer qs from stem pr just lucky guesses.
Well usual life guys, feeling better today, now i appreciate how awesome it is when you feel fine physically.. So let's enjoy it..

Although never done binge drinking i know exactly what you are talking about.
I have BP of 90/70 every once in a week. Yesterday was that once in a week. Feel much better today. Sitting and studying is like gambling if you dive deep enough, you can hardly stop doing it although it gives us headaches and bp drops same story as with step1.
But thanks god no complaints this is easier and at least interesting.although we are ready that exam will be harder, 9hour race..

So for summary everything goes by plan for now, will finish uworld by december gonna do one good review of mtb also by that time and will write NBME.

And Im binging on candy now!! Got a lot of candies for Halloween, but not many turned up...Treat for me!! The best part is, it doesnt give you a headache.
Will finish peds today..Can someone suggest an NBME to take at this point..I finished all the subjects one time...Not done much of USMLEWORLD (may be 10 blocks, without the explanations)..Thanks a lot!

Ohh don't say i will need chocolate detoxification soon. I can't do a block on good percent if i don't take morning dose of chocolate. I will start to hate it soon.

I also want to know which Nbme to take first. I.e. Which is the hardest or more accurate for exam to keep it for the last, before he last last self assessment which is UWSA.

Maybe this poll will help us.
http://www.usmle-forums.com/usmle-st...-ck-score.html

Also the correlations thread has the sequence in which guys did their assessments.

Looks like NBME 4 is pretty close to the actual test..and NBME 6 is the latest..I think its better to do NBME 4 and 6 once done with the q bank.

i'm still in my first subject IM...3 books taking too much time. :/
Am i lagging way behind? how do i speed up? I have half of neuro and emergency med left before i can wrap this subject up and move on to others. Can you tell me how much time the remaining subjects will take considering i am doing them from 3 books?!!
I do want to take this exam by feb 1st week anyhow!

I think first assessment should follow after finishing u world and couple of days revision of mtbs. We should prepare for it as for exam.

I agree about nbmes rockstar i will do 4 when i finish u world in the end of this month, then 6 and then uwsa in the end.

Usbound i would recommend to give one read to obgyn and other subjects from kaplan and use both mtbs for them. Ive been doing minimum 50pages a day so 2 weeks should be more than enough. For all three subjects. Then start u world and along with doing that revise the things you find yourself deficient in. Don't suffer about things that you don't get from first read in either kaplan or mtb. You will meet that exact subject very thoroughly and nicely explained and charted in uworld.
I agree three books take hell lot of time..

Good morning and good day guys..
Started another hardworking weekend.
Use it well

Okay, since i finished all the subjects once, I think i should do the q bank now..Im starting with reviewing all the blocks i did before..That should take about a week...Im still not sure about the NBME..I was hoping it could motivate me a lil more to study, but let's see...Does anyone know how good is step up to medicine is? I got super excited about step 2 and bought all the books out there..My husband's on call this weekend, so I might do atleast 1 block today...Good luck and good day guys...and please keep posting all the motivating stuff..That's what gets me out of bed everyday and Im not exaggerating it.

I got super excited about step 2 and bought all the books out there..My husband's on call this weekend, so I might do atleast 1 block today...Good luck and good day guys...and please keep posting all the motivating stuff..That's what gets me out of bed everyday and Im not exaggerating it.
All you need now, so do need i, is thorough doing of mtbs (both) and u world. For next month or so. Nbme will not help us now. Will do it on low score and get more unmotivated. So lets work slowly with patience and with the plan we got snd should follow it. Step up is good as guys say, i have done couple of chapters, some people use it instead of kaplan notes , but i prefer to stick with mtbs for now. Cause there are lot of things in it that i don't remember. Like tumor lysis syndrome electrolyte disturbances. Missed it today. So i prefer to do this book couple more times than anything else. Im very sad now but it will pass and tomorrow morning ill be just fine and work as usual.

So I will start UW only on 13th Nov; Planned to finish it in 3 weeks by any means possible.I have to take the exam not later than mid January. Come on guys lets conquer our destny.

I also can't take it later than mid january or i will go crazyyy
Count at least month for uworld. You should do it thoroughly. And month will be enough if you do at least 2 blocks everyday. Believe me that is difficult, although doable. Im doing it for 2 weeks so far ---> 22 blocks. Will need to prolong one month subscription i guess. Because will have only half days for study for next two weeks. Kind of resting today and trying to keep myself organized. Did one block 73% and just incorrect ones and left overs from yesterdays second block. Am very reluctant today but i know i'll be good tomorrow.
Lets keep each other bumped up, you guys are really helpful, god help us

Arranged surgery notes and did surgery from mtb2, comparing with mtb3.
Things that are in mtb2 are far well written than mtb3 abdominal surgery for ex, although it totally misses cholecystitis. Also it missses pediatric surgical things but they are discussed in pediatrics. Takes lot of time, comparing and still cant decide which book to take. I definitely disagree that other subjects can be done and revised from Mtb3 only!!!
Congenital heart dx written way better than in mtb3
Above mentioned is true generally about every topic in other subjects than IM.
Conrad we love you so much, you should not have done this to us
Good night!

Good luck guys, doing left over explanations from previous blocks today, also some hospital work. Tomorrow starting 14 days of mobilisation. Early morning one block, then training untill 3 pm, then doing explanations untill 21:00, then reading mtbs and rearranging annotated stickers untill i fall asleep on ma table
December is our hardest month!!!

Medicine doc i can't memorize them, just be aware when to do radical operations. What i do memorize is lymphoma staging

Rockstar i have discussed with you the housecleaning stuff, we are tearing apart to have a nice profession and someone else to be busy with home stuff. If this will make you feel better i also cleaned one evening after finishing studying, one evening somewhere in previous week and not going to loose daytime on that again until new year.

Good luck work hard, forget that outside anything else exists, be obsessed with study and this will end soon hopefully, with nice scores

Correct Answer

I was away past 3 from study due to family emergency. I start to miss the simple rural life; having farm, animals a clean air to breath. Have few beers in the evening talk about politics or football and cuddle my wife and sleep.I wish that but may not be in this life;may be I have to save money for the next life as a doctor.Back to OBS/GYN.Hope to finish it tomorrow.

So
Start Pap from 21, despite onset of sexual activity.
21-29---> every 3 years
30-65---> every 3 years cytology, or every 5 years cytology +HPV testing.
>65---> no screening if recent prior tests negative.
No screening---> if hysterectomy for benign causes (i.s. no previous history of precancerous lesions, cervical cancer, or DES exposure)

ASCUS----> 21-24----> repeat cytology in 1 year
>25 -----> HPV testing---> if positive do colposcopy, if negative- repeat Pap and HPV testing in 3 years.

MTB sais do repeat Pap in 3-6 months and order HPV testing.
Had trouble with this in question, cause they gave both as different answer choices.

SO it is clear that if 2 consecutive Paps show ASCUS, or HPV16 and 18 are found or ABNORMAL Pap ------> colposcopy and biopsies.

Endocervical curettage---> all NONpregnant undergoing colposcopy for an abnormal Pap
Cone biopsy---> if Colposcopy and ECC are not consistent (i.e. abnormal cells were not biopsied) , abnormal ECC histology, endocervical lesion, biopsy showing microinvasive carcinoma of cervix.

Observation for ----> CIN1, and CIN2,3 after ablation or ecision
Ablative (cryo, laser, electrofolguration)or excisional procedures (LEEP, Cold-knife conization) ----> CIN2,3
Hysterectomy----> biopsy confirmed, Reccurent CIN2,3

Pap in pregnants managed same way except ENDOCERVICAL CURETTAGE.
CIN/Dysplasia- pap and colposcopy every 3 months during pregnancy, repeat Pap and colpo 6-8 weeks postpartum. If persistent lesion definite treatment.
Microinvasive cervical cancer---> cone biopsy to rule out frank invasion, deliver vaginally reevaluate and treat 2 months postpartum.
Invasive cancer <24weeks---> radical hysterectomy + radiation
>24weeks---> conservative management up to 32-33 weeks, cesarean and definite treatment.

Classification of Pap Smear:

ASCUS----> intermediate smear
HPV, Mild dysplasia, CIN1---> low grade squamous intraepithelial lesion LSIL
Moderate, Severe dysplasia, CIN2,3, CIS---> High grade HSIL

Cancer: Invasive cancer (management: cervical bipsy, metastatic workup----> hysterectomy. if mets, tumor>4cm, local reccurence, positive margins---> Hysterectomy+ adjuvant therapy radiation or chemo)

HPV vaccination:
Quadrivalent HPV recombinant vaccine (Gardasil)---->8-26years
Sexually active women can also receive vaccine but in above age range only
not recommended for pregnant, lactating, or immunosuppressed women, also recommended for men same age to prevent Genital warts. Vaccinated ones should still follow with Pap smears.
Its a great summary. Thanks!!! Now everything makes sense!!!!

Thanks for reminding me, have to put this summary in my blank page of mtb2 in the end of gynecology.
I really worked on this to have one good summary.

So did an evening block 80% . Moved one percent up with my cumulative 71% so far. UW is not easy. Quite tricky. Afraid if exam is way harder than this

Your posts are quite motivating.Looks like you are working hard also u have two kids. After doing step 1 with a handsome score I actually on the wrong track for a while. I thought I can do it with shortcut like reading MTB and USMLE WORLD Alone.

I really don't understand what the exam are all about. Is it bunch of USMLEWORLD prototype qs which I am good at . But, if it is really like u need to know the concepts , then its lots of thorough studying. I just ordered Kaplan lecture notes.

I at least want to go through it once and refresh my lacking concepts.

The main thing is u need to speed up at the end and revise as fast as u can. Otherwise u will forget.

Did, u finish Kaplan q bank, What ur average in that.

I know you will do very well.Keep posting and happy reading

Thank you for motivating..
Yeah, speed up is the main thing. We need to accumulate lot of info and burst it out in 9hours, it depends on our energy, performance, mood, fed state and endurance how we will perform.
Today i was telling my friend that before step one i was not afraid, had some feeling of unknown impeding doom but now i really am afraid, cause i am ready for more difficult, ambivalent, ambiguous exam, during which i will not have time to think. this really scares me.
But we are better then that.
I did kaplan bank, did not do explanations just quick read through correct answer.
Average was 67%. Those questions are harder, and not high yield with regard to their importance as a study tool. Although doing them really helps to workout time pressure.
You should give quick read to kaplan notes but main thing to stick with and cram will be mtb, annotated by you. You will see difference how you will read mtb after doing and along with doing kaplan, it will give emphasizes and you will add nice charts explanations from uworld

Good luck. Thanks again.
Good night guys work hard.
As my best teacher said: make the plans, stick with them, realize them. It worked for step one, so we gonna make it work for CK too.
That's what we are doing daily.. Good luck god help us

I agree with you. If We could made it through step 1 , I can do it for step 2 ck too.Last night , my family was kind of bursting over me.

I have a kid too. They don't understand how we slowly build up our knowledge and concepts and work through ambiguous questions every day.

Today, my energy is so low, feeling guilty.Have all the SIGECAPS symptoms of depression.

But, I know tomorrow will be better.Just want to feel happy.

Only those who go through this can understand us. So lets keep here together, "usmle psychotherapy social support group "
Today i started explaining what steps i need to go through for ecfmg certification, and during half of my talk i was angry with my self why was i losing time and my nerves..she did not get any idea what are we going through even if she understood everything. So don't get angry on anyone.. But make everyone appreciate what you do. And help you from their side, which means only one thing: to leave us alone and let us study

Just came from training, also was Sigecaps positive during 6 hours, unrefreshed from sleep, hardly accomplished simple tasks. Although working with laparoscopic instruments is quite difficult esp. For 5-6 hours. Drank tea, took breaks. Didn't help much.

Good luck everyone.. Keep the work so we go through this successfully. Then god knows what will happen with us.. we have to finish what needs to be done from our side.

I love cereal with milk, that is my and my kids' favorite "food"
Will enjoy it and start todays hard work. 16:22 will be great if i go through one block of explanations from yesterday.

Oh yes honey, you are in trouble. Trust me and don't try to make one book. Or you'll go crazy and will need a month to do pediatrics. And still you will come back to read both mtbs in the end.
Good luck with your new trouble, you will see how easy your life was before

Now we have a perfect group: girls complaining about husbands and guys about wives
You are asking that question to an wide open space i hope, who has calm life with family.. Do you remember when you woke up just by yourself, without anybody awakening you from your sweet sleep?
Sometimes i am jealous of anyone who can sleep, act and live alone by themselves.. Did 10 explanations am really nonproductive

I know what you are talking about;I have been there twice. What I have learned was "I should listen only what I want to hear". All the other times I pretend everything is a background noise.

felt myself as an old lady after your quote. Im just fine but need to check bp when not feeling okay. Little bit exaggerating about weight. Its just killing me that i can't be in usual shape due to this studying hours. So that's why we need to finish soon. Lets go girls and boys no more complaints.
You are doing better see your comments are showing y get fruitful and working more. Keep like that, have a habit of finishing daily plan.

Im so sorry...I didnt mean it like that...I gained a few pounds (10lbs to be precise) when I first came to this country last year...It was hard to lose the extra weight while studying...Been there, done that...So I can relate to it.

Good night remember always hypothyroidism in differential for : amenorrhea, depression, myopathy!!! and Secondary hypertension.

Also for secondary hypertension: remember differential
renal vascular dx,
renal parenchymal,
primary hyperaldosteronism (which is by itself highest yield to remember woth: LOW renin, hypoK, slight increase in Na, metabolic alkalosis, hypertension+adrenal incidentaloma),
Pheochromocytoma
Cushing's
Hypoparathyroidism
Coarctation

Always consider econdary cause and do further evaluation when: young age of onset<30, resistant hypertension requiring more or three drugs, severe malignant hypertension.

Welcome back to game
You definitely are not lagging. Just no more missing days from work
Everyone here can give their summary what's done so far.
Mine is: first read of mtb2+ kaplan q bank except emergency section, without explanations.
Quick read of kaplan notes all subjects, + mtb3 for other subjects with so-so annotation of mtb2.
Some experimenting of coalescing things from mtb3 in mtb2 for other subjects, which i would not recommend to do, except you could duplicate things like pap workup, and some other high yield stuff. Must read both books for other subjects.
Half of uworld finished 25 blocks today, average 71%.
Should finish uworld by november, do one good but quick review of mtb and generate my first Nbme.

Others also can do summary, where they stand.
So myfriend the only thing you maybe were lagging was motivation. Keep yrself motivated with us advertising thread.
Now really good night. Finished the other half of block anyway

Good help us

77% today, after 5 hours of laparoscoping. Did apendectomy and cyst enucleation on glove filled with water it was fun.
I have not done vignettes in surgery and will do them after finishing todays block of explanations. This is a shame in january i will be twice certified surgeon and my lowest performance is in surgery
Also how many times should i miss analgesic induced papillary necrosis and also these 50/50 choices kill me.

Let's keep each other motivated guys. This thread also helps me a lot, cause we are like little support group here. And those 5,000 views are not only us, so our progress motivates others too. Keep it that way

Sorry to intrude. I've given my Step 1, still waiting for results. Just gathering my stuff for CK. I thought it would be a good idea to gain tips from you guys who're already in the middle of their prep.

I'm planning to do Kaplan notes with videos + MTB 2. How does it sound?

My queries are -
1. MTB 2 vs MTB 3. I can only do one as my main focus would be on Kaplan notes. I'm inclining towards MTB 2 based on reviews.
2. What about Kaplan videos. Are they recommended? (I'm a huge fan of all the Step 1 videos except Pathology.).

Any inputs would be highly appreciated.

There was once a guru named Conrad Fisher. His students were lazy and tired of reading big reference books. He gave them the easier version in two books and named them MTB 2 & 3.The disciples were happy but the later books were never complete. They had to go again and again to the first book. Finally the end up reading 3 books.I tell you Conrad; for what you did to us, your next life is not among humans ...

There's no intrusion, you just barge in (that's what i did atleast)...I think KLN videos are good for IM and OBGYN...I watched Peds, but didnt think it was worth it...I didnt watch surgery videos bcz i was told they were no good either...As for MTBs, I think you need to do both the MTBs...If you do KLN notes, MTB1 and MTB2 together, you will see that a lot of topics are missing from the MTBs..So its good to do all of them together..This is my take on Step 2 ck prep..Good luck!

That's what i am screaming during whole this thread. Why did he do this to us
DNA you have to do all three of them, main focus on mtb2 for internal and add mtb3 for other subjects. (Minimum other subjects, i would still go IM also with mtb3 once, to have no doubts left that i am missing something)
Give one read to kaplan, a quick one, about videos i am skipping them. I am too angry on conrad to listen to him having fun while teaching. I've lost enough time running through these 3 books in and out. But for the beginning you may try them and act however you feel about all these materials.
Good luck, main thing is work hard and don't lose speed from step1. You are fresher then ever. This is you major advantage.

Hey. Thanks for your response. Yes I'm trying to get in the game ASAP. I just don't get the point of writing two majorly overlapping books (but I guess I do after all). OK, I'm a vicious reader so I'll try to skim through the 2 MTBs while building firm ground on the Kaplan notes.
I do get your point about Conrad's videos. Sometimes you do feel like punching him through your laptop screen. I'll see how it goes and do as time permits.

Lots of luck to you guys.

Good early morning, 6am, 80% today. Going back to sleep calmed down

NBME, UWSA, Qbanks Averages, Correlation with Real Step 2 CK Exam Score

I use this thread whenever i have questions about percents. Kaplan is harder, but it does not necessarily mean better then u world. Percents are usually 10% lower.
