Same here lot of social and family baggage, about personal am somehow anhedonic..

Good day people!!!!! This surgery is killing me!!! its way too much boring Its kinda slowing me down!!!
Not happy with my day today!!! Wasnt fruitful enough!!! Dont know what m i doing!!! This is no time to waste!!! and i ruined the whole day!!!!Terrible!!!!
Hope everythings going fine at your side guys!!! Stay motivated !!! Thats what will keep us going and help us reach our target!!

endocarditis prophylaxis-confused

contradictory info in mtb 2 amd mtb 3

mtb 2 says,

only in congenital cyanotic heart disesases- unrepared

mtb 3 says,

in any congenital heart diseases

why Conrad wrote two books and two books are significantly different. just a marketing strategy.

MTB2 is more current with new updates
so thats the way to follow the guidelines

Hi guys! Hope every body doing well and working hard!!
I am going back to ophatalmology again.. I am doi g wrong on this almost always..(
I am glad and sad same time...( sound like bipolar loool) finaly i got observership ( glad for that) but less time to study which is so sad.. I did timed UW qs first time and got 64% is it bad?

had the lowest performance in ophthalmology

I dunno how you guys do it, but I definitely dont have the attention and focus like you guys..I didnt do much today...watched 2 movies.....But you guys are doing a great job..Goodluck and goodnight guys!!

66% in early morning block, 80% in late evening block. Will do explanations for both tomorrow
So i lost todays day because of laparoscopy and my eyes heart really bad.
So i suck at surgery, 60% lowest performance, because i have not done vignettes!!! The first part of surgery in kaplan is total ****. So first thing to find out time for iiiiiissssss Surgery vignettes.

No movies watching no TV, no you tube, no eating no drinking no moaning, no complaining only working at our weak areas, and doing and doing our best.

Welcome. Keep yourself indulged with us here and you will feel responsible for daily hard work and feel guilty for any lost time.
I have bets performance on psych, "my" step1 first aid is the best, i.e. Annotated by me. Have done step1 kaplan twice in my past life. Have not done anything yet for ck, but psych and stats >80 are compensating my low performance in surgery so far.

Rockstar stop watching movies and punish yourself with two consecutive blocks and explanations.
Krazy don't worry u ,ay just got tired because you are following this thread day by day without interruption. So just maybe go to vignettes, as i know they must be little more interactive and interesting
DrAga good luck with your observship!! Go to correlations thread and see how uw 64% correlates with real score. I generally aim to finish minimum 70%, to feel satisfied, but for sure uw need to be done again. So you will improve your knowledge, percents are generally way of feeling better about yourself. Main thing is to work work work. Don't slow down because of observship, give yourself daily plan and follow it, or you will lose focus. I am getting very tired, my eyes hurt because above whole day reading on ipad now i have 5 terrible hours standing and watching very eye irritating laparoscopy i really need to rest my eyes. But this is just for one more week. Trying to hold on myself. After that i plan furious attack on CK.
So what I'm saying just stay organized focused and losing minimum time, sometimes we may sit and study but still lose time just by simply thinking about something. So cut down all distractors.
Good luck and goodnight everyone. Thanks guys for being here together this really helps.

So let's use everything for our sake guys, harder preparation harder we should get and still be prepared hard as rock, and rock CK. Nothing will be lost..

Same here, could not do more than one and half block of explanations. But that is totally fine i think.

Im with you guys.. Today's hardly achieved 61% block which was so full of opthalmology cut down my performance in opthalmology to 51%. By the end of block i even thought to pause the block and read opthalmlogy in mtb and then change answers
Okay doing this block's explanations and reading opthalmology and my stick notes i need to go back to mtb reading or will bring my scores down. This week which i've been studying only one block of explanations and nothing else had impact on my performance generally. So need to finish this training soon and from monday im in usual wolfy shape of whole day, lot of hours studying

Good luck guys, some guy on match forum posted he would be glad to work in surgery residency unpaid, if they give him chance. No thannnnnksss, i have done not only unpaid 5year residency in surgery in my country but i was paying for it every month.
Okay. Lets think about the best.. And do our part in this war..

Hello all. long story short, i was studying for step 2 ck for about 6 months until i suffered a disc herniation at l4 and l5 back in august, and had the surgery for it to correct it last month ( I am fine now if anyone is wondering ).

So basically i have been out of commission for about 3 months now, haven't touched a book…i have taken 2 nbmes (just missed passing by few marks) and have completed UWORLD with an okay average (53 %).

Now its time to get back on that horse that is the usmle. Done with CS and Step 1, need to rip this exam apart!!!!

Will be updating daily

starting off with something not so hard (preventive medicine)

Congratulations with getting back to shape. Good luck, rip it apart, so you had positive straight leg raising test yeah?
Post and keep updated you should be done soon if you have gone through all the material.

P.s. Preventive medicine could be quite challenging on exam and nbmes i've heard. We could make some summary here. Mtb+ UW summary

WELCOME BACK TO STUDY!..i am sorry about your herniation but u can find good thinks on it...SUBLIMATION REACTION--bad experience turned good on something else( we have a lot of qs about back pain at least u can easly differentiate herniation from compression for example)..

2 blocks 73 and 75%.
Have to do explanations of them and also half of left over block. But no time just after 6pm. Impossible but let's see

Thanks a lot, glad I'm finally back to some sort of normalcy.

Although i can't lie, i did enjoy watching all the tv and movies I could in the last three months

Yeah preventive medicine can be a bit difficult but its all about the repetition, getting those numbers engrained into your head, finished the chapter yesterday will review later in the week to make sure it sticked

MTB says "If ACTH level is elevated and doesnot suppress with high dose dexamethasone test, scan the brain with an MRI"

Above statement rules out pitituary cause of elevated ACTH so why wud u wana scan the brain with MRI????

Good point just paid attention.
Only thing i can think of is: i guess it means pituitary source if it is suppressed.
When it is not suppressed it still does not rule out pituitary cause..

Good morning guys!!
Doing explanations 3 blocks to go reading surgery vignettes along the way. My surgery performance went into 59% would be better if i have skipped first part of notes and done vignettes from head to toe. But its not late.
Good luck, and work hard guys.

For our indian friends: i live very close to Medical university and in neighborhood there are so many foreign students mostly from India. This morning one of them knocked at my door saying move your car away just wanted to say that they are very integrated in my city already. I even have cafe, shop with outfits in neighborhood

it questions the credibility of high dose dexamethasone suppression test thou! well i think its a mistake n assume it to be suppressed by dexamethasone test and then go ahead with MRI to find an pitituary adenoma.

I too broke my head reading that line long back! Its a mistake!
Elevated ACTH = Pituitary/Ectopic (lung)
Pituitary suppresses High dose Dexa and Ectopic doesn't..
If ur sure its pituitary(cushing's disease), scan the brain, locate n remove the lesion.. simple

Even I plan to finish First read by this month end! Had only done IM. Done with Psy from Kaplan LN, Mtb 2 n 3, First Aid step 1 n OB videos n LN almost over. Gyn LN gonna start tomorrow no videos though! Gotta go faaaster

I guess uwsa around 230 equals to score less then 220. So i swear i won't take exam if uwsa won't be around 250. Not because my goal is 250 for sure. I have to get above 230. So uwsa around 250 might be considered safe to get score around 230.
Conclusion from already a lot of experiences

yup. agreed. very scary.! Nothing can assure a good score . if we get lucky on the exam day, uwsa becomes predictive or underpredictive in some cases. a bad day will ditch us. lets leave everything to god and hope For the best.

UWSA is evidence based already good for us. It under predicts in lower scores, have heard already about 20experiences like that. Its quite accurate around 250 scores.
So good for us we know how to move on.

Uw sais baker cyst in osteoarthritis, mtb sais rheumatoid arthritis, what's wrong with all these guys in different books. Can't they agree on something

happy studing everybody.
I got my Kaplan lecture notes. Started with with GYnae and obs. Problem is nobody start a comprehensive book 2 months before exam. I Already done usmleworld 3 times. USMLE RX almost done with 82 percent average.Now is the time to cram and summarize. I got stuck with this huge book. But, I have to go through it topic by topic , subject by subject.

then revise usmleworld again and take assessment test.

Guys I need your prayer.

I think it doesnt matter when you start a book, you know better what you need to focus on, so its your decision...If you are already done with the q banks, why dont you do an NBME? That way, may be you will know better where you stand...How much was your uworld average? That can give you an idea too, right?

have a question does somebody know which NBME in USMLE.org as a free for download?..i have done one from USME.org got score of 78%=530 on nbme==223 on usmle.....but dont know which one i did...i think its old one..cause i dont think its my really score.real probably will lower then this

84%, but my cumulative is at steady 71%. Did one and half block of explanations planning at least one block wit explanations until sleep time.

What's usmle.org Nbme? Is it a different nbme than nbme.org nbme?

God bless you do however you consider is better for you. If wont lose too ush time and do kaplan read quickly that's quite good, youll have kaplan notes fresh in mind. Good luck, i would also think about writting one nbme. But you don't feel ready yet, maybe you finish kaplan read revision and nbme. I also plan to do nbme one and half month before exam, when i feel more or less prepared after finishing uworld

the one you did was free 150 questions. Those are not NBME. For NBME you have to pay.

Getting 78 percent correct is good.But, I really don't know that correlate to a specific score.

Nbme forms are better predictor and they will give you a score prediction report report after you finish the exam

hi everyone i finished uw one time with 69% with 70% of wrong answres reviewed study only form kln which i finished about 2 times got 234 on uwsa 2 month ago and 235 on nbme6 one month ago and now i am reviewing kln again , its like keeping water between your fingers ,my target is 260,i got 246 on step 1, i dont know what to do?

15 blocks left in uw. 82% and 73% todays early morning blocks, am getting in the shape of panic mode. can't sleep so easily anymore.
Starting super preparation from monday. From morning till night. Most important month(s) ahead---> what an d how i will review everything in end november and full december will make my exam score. So I'm preparing for preparation.

Really? Cause when i open the bloks its Was named NBME.. But it was more then 150 qs.. Were 4 bloks 1-46,2-41,3-41,4-11. I really confused..

If they did not give u a predicted score then its not nbme forms.

Start with vignettes, or the dense and very uninteractively written first part of surgery notes will make you less and less motivated, i did the first part and left second one, that's why am dropping my uw score because havenot done what was most important from surgery notes, but lost time and motivation with the worst first part ever..

Have done couple questions of exlpanations today, around 17 questions, watched very nice translation of operation from france, slept couple of hours during day, cooked and ate my favourite healthy fresh chicken salad, with cucumbers and tomato and fresh green salad, now reading some of my favs of russian, french and english poetry on the couch and will fall into a very sweet sleep..

Im doing the vignettes too...Its so much more interesting to do vignettes...Well, just when I thought Im getting a little into the usmle prep deal, my husband is dragging me to visit his family..I know I sound mean..But i just wanted to study without any interruption..So, Off to St.Louis..Will be back on Monday and will finish the vignettes on Monday..In the meantime, good luck guys!!

Good to see everyones studying going well!! I had to take a break because had some family over to visit, so haven't done anything the last 3 days :/

Still have to finish dermatology and then i will review it along with the preventive med i did earlier in the did

Default UTIs

Cystitis
Uncomplicated nitrofurantoin for 3 days, tmp/smx or cipro
7 days if anatomic abnornality, if diabetic.
Quinolones not used in pregnants.

Pyelonephritis
Drugs against gram -
Ceftriaxone, penems
Ampicillin and gentamucin until cultures known.
Cipro oral or outptient. After 48-72hrs oarenteral therapy when uncomplicated and responds well can be switched to oral agent according to susceptibility.

I had kind of gap in study last two weeks only could manage to do one block with explanations, and i did start to forget things and get slower. Only tip you need is start slowly and achieve the speed and study hours that are "normal" for well preparation for this hard exam. Of course you won't be as fresh but believe me all the basics are in you if you have done step1. So go for it!!
So did 2 and half blocks today. 7pm starting mtb revision goal 50 pages lets see. Next days i think it is wiser to review book in the morning.
So good luck guys and i don't hear your voices hope you are not cheating on CK and watching some movies or having fun
I will have scheduling permit next week
Kinda have temptation to finish exam by christmas..

I previously went to one proper read of Kaplan LN and then skimmed thru highlighted points. I did MTB 2 twice. For Paeds I just went thru MTB3 as its Kaplan LN r poorly written.

I am also doing a job but plan to study 6-8hours?day. I want to coordinate with u all! Hopefully v'll ace this exam!

Looks like I am getting Dumb day by day.How angry do u feel when u read a topic then forget the key points the next day.

Doing Orthopedics from Kaplan , I see few discrepincies .

Acute osteomyelitis In Adult ,

Kaplan says you need to surgically debride the tissue then give antibiotics

which makes more sense

MTB did not mention surgical debridement.

Got a funfact , from Kaplan.

I finished todays plan honestly, 2 and half blocks one new, one old, did infectious and immunology from MTB2. We may complain a lot but this book is good, maybe reading it smoothly without other book made me so happy, or maybe cause i have not done it very well on first read
When i finished infectious wanted to quit today, but i had to post on this thread how i did todays plan so.. This made me do another ten pages and step1 first aid for HIV stuff. So i also do love this thread..
Can anyone remember regular 3 drug regimen?? Emartisomething/efavirenz/ can't remember third one just read those

Its normal to forget things. when i looked at my step1 book which was my best friend for months and the pages and manners where and what is written i still remember when answering some questions even--> even this book looked so foreign, thats why I'm trying to do everything quickly, sooner then i forget the stuff, couple of times etc. revision revision revision.
Debridement is not the must thing in osteomyelitis, u my even not need it. Biopsy is just most accurate to diagnose, otherwise in kid as in adult you may just cure it with antibiotics. Depends on severity.
There always are things written differently even in mtbs itself, but don't get to nervous. Main things basic things are clear and clear cut, they won't ask what they themselves are not sure about.
So just move one move on, the fields you are weak in, reluctant to do will be done better on second turn. So just study well guys, and study a lot during the day. What are we complaining of compared to step1 bullcrap, this is at least clinical medicine and interesting..

the good thing u did ,Read the Kaplan first and the q bank. Now, doing the cramming. I did the opposite thing . Did cramming first and going on more details.

First , I am enjoying reading Kaplan actually.MTB3 is summarized version of Kaplan in most of the topics .

Default Uw performance decreasing! :(

Guys I wonder why my uw performance is decreasing day by day. I was at 71 but now its 69%. I understand that these scores really don't matter but they are discouraging. Got my lowest score in the block I did now... it was 52%.

Explanations of 5 blocks are pending. I think I'll finish these explanations and then do 8 blocks in a go. Lets c.

Keeping my 72% with teeth. I also got one 50% in early blocks. You wanna simulate exam with 8 blocks? I eanna do that also have left just around 10 unused blocks. Hardly doing explanations of 2 blocks during the day
Ha what do y think should i give a try. I also have an idea maybe i should keep some unused blocks by the end of exam?? Maybe i should write nbme earlier slow down uw and apeed up mtb revision??

My friend got 72% in UW and she got 250 in real exam. 72 is great!

Simulating like real exam is a great idea but its not necessary. U can plan the breaks duration when ur simulating. I simulated for Step 1 also, only to realize that I've stamina. so I was relaxed in the real exam even after my break time was utilized fixing the computer which crashed twice for 5 mins. I recommend that u def give a try.
Ur strategy sounds good. U shud slow down on UW, go thru MTB and maybe write NMBE earlier. U can also write 2 NBME exams in one go to simulate like real exam.

In addition, make sure u adjust the brightness of the screen. I experienced that if u increase ur Eye blinking freq. ur stamina increases

Im taking care of my eyes, using protective eyeglasses although thanks god don't need correction of vision. Blinking and stamina correlation sounded funny
So if i simulate with uworld i will finish it in one simulation so maybe simulating with nbmes sound better but expensive idea. Step2 is all about time and stamina so i will think about that.
Generally >75% is kind of guaranteed >240, but of course everything is individual. My friend had 73- % got below 220.. So lets see how it goes will definetely need to make quick quick revision of mtb. Good luck guys. Have not done any reading today, only two blocks. had something more important to do with my friend's kid's health problem.

November 21. Have permits on my two exams, home licensing and CK.
Decided to save at least 5 uworld blocks unused. I will simulate exam with nbme4 and uw blocks. Want to pass it soon. I guess i aim december because i will have very serious preparation next 4-5 weeks and i think i will be in my best shape by christmas aluhng not hurrying and jan 15 is still the destination point, its all about stamina which is at its height at the end on serious preparation.
Already Two of my recently passed Georgian friends said that qs are more or less like uworld, so i won't kill myself worrying anymore that questions on exam are something unbelievable as lot of other people say.
So let's rock it. Will do mtbs most of the day. Will do one good revision in next 9 days along with one block of uworld untill i have left 4-5 blocks ----> then simulate exam with NBME 4 on december 1

Guys I am not that focused. I need motivation!! I am doing a job and also helping mom doing groceries.

60 days left! A lot of Pending stuff. Need a proper routine.

Do u guys think I should quit my house job for next 60 days? my supervisor says u can get 30 days off, from Dec 21st onwards and we wont cut ur salary however, 60 days is not acceptable. They want me to b in hospital for only 4 hours everyday for next one month.

For 3 weeks i have changed my routine whole day studying to study after 4pm and it was terrible, i went of the rails. Trying to put myself and old routine for these couple of days. If you can not take more than a month then take last month off. Last month should be off as possible. It is hard to give an advice about quitting job, i took off from one job and they kicked me from another and i am so greatfull to them

So, finally I am doing study like this Musculoskeletal+ Padiatric Muscoloskeletal+Orthopedics - like making a combo of medicine surgery and pads from Kaplan , mTBS and Lecture and usmle world qs on this topic.

Subscribed to USMLEWORLD Again. After 4 months.Did a block got 4 incorrect only.not as bas as I thought.

Need momemtum and confidence .No matter what always belief that you can do it and enjoy and only enjoy ur study
