My problem is I included kaplan lecture notes 1 .5 months ago in my study schedule.Before that used First aid and Usmle rx. I actually started my study with random qs from usmle world even without reading MTBS well. I think I read bit and pieces from everywhere and my content is not well organized. Its getting me serious hard time.

Kaplan paediatrics huge and lots of information.I am just using it as reference and focusing on the topics that are in mTBS and UWORLD.

Am I doing the right thing

When i started the q bank, even i didnt do MTBs..But i like to go back and forth between MTBs and UWORLD..This way, i find it easier to see what's imp and tested...But whatever works for you...You should take an NBME before speculating

I seriously think you are stressing out over USMLE tamta...I like your speed and energy though..I wish i were like you..Im sure you will do well on the test regardless of UWSA scores..You have been consistently doing well..Im kinda getting nervous now..I hope i dont get carried away by this NBME and do well on the next one too.

Inflammatory condition so better to avoid anything oral.

Moreover might need surgery so better to keep
NPO.

Rockstar this speed and time pressure is needed to do this exam and finish it soon, yeah i am stressing myself probably, but hopefully this will help on exam with time pressure and long exam day.
I will relax after uwsa. Hope it will be above 240
I could very slowly persistently thoroughly do every subject with kaplans, videos, both q banks, uworld twice...i would with grat pleasure do step1 behavioral again and would enjoy it.. But i don't have ability to do that. Need to get back to normal life soon. I have outgrown my emotional, material, familial, financial blood supply..
I also think that it may be as efficaious to do exam in two months of inhumane study than 6 months of medium level anxiety study. And those 4 months make a lot of difference for me and my family.
Be persistent and work hard. I guess how many times i didnot do mtb it will be not as difficult. But will definitely need time as it is for uworld. I am whiner here today but did around 130 pages: pulmo, half obstetrics, derma, preventive, cardio MI part, rheumatology.

Will start with neohrology tomorrow+ congestive part of cardio, infectious, endocrine... Shock stuff from surgery.. Don't think ill be able to do more.
And of course one block of uworld as usual.
Did half this morning now will do surgery 24qs until midnight.

How many times are you done with mtbs. So many rapid reviews are you doing ?? How can u read the entire syllabus in 2 days. Can you give some tips because I still have 4 weeks left and I wanna make sure even I revise mtbs twice atleast. Along with solving uworld second time.

You did not answer my question. Did u think reading Kaplan paediatric is necessary. There are some topics that were were not in mtbs , I read them from Kaplan.

but too many things like intorsion, too much details in jra you know what I am taking about

Yeah, a lot of topics are missing in MTB..But i believe they are missing for a reason...I think Conrad fischer has included only tested and important topics in MTB...I did read it the one time i did kaplan notes...but not the 2nd time...For revision, Im sticking to MTBs and referring to kaplan for a few topics

Got 248. It was so tough i though i would not make it. It was same feeling as guys say coming out of exam. It felt like real exam. So im glad i dont have to postpone it.

my friends are asking me what helped and i will answer here.

only think i can think of helping is INTENSITY of PREPARING AND STUDY.
i know what helped the thing that i could not find any cloth that would fit me well because i got so fat not because i eat much no because I DON"T MOVE I Sit like frozen for hours 8-10-12-14 hours. from 9 morning to 23:00 00:00 and average i study 10 hours. because sometimes i may just sit nauseated and hypoxic or hypotensive.

we should do this step wise accelerating i still missed qs about thing that i dont understand like Incontinence although i dnot miss all of them this time as before on NBMEs.
i also had bad performance in hematology which i could not review in this last time period and marked questions of which i have not done. and i knew i wanted to review it but i could not.
also i didnot do arrhytmias although did better on cardio this time. although again low performance.
three weeks apart i improved 13 and 14 points up it means acceleration review and revision helps.
the other point is hoe many times we didnot read we still miss such things oh god... wanna kill myself. having trouble with SICKLE CELL questions, CARPAL tunnel questions and generally such soft subjects that we are so aware of.. so its state of mind. and we need to get in that state of mind where it does not matter how many more blocks you are left. ive been shaking last two blocks freezing dont know why and doing questions like ROBOT exactly just highlighting and never even looked back on highlighted ones. so i hope exam will not be worse than this. although if i can imagine twice as long and plus drug adds questions OMG i will really need help of god. don't know why so freaked out. but hopefully this UWSA score will be the only thing that will help me stay calm the three weeks of waiting for report. cause i know exactly now how exam will fee. ambivalent not sure about half of questions exactly as other friends said i was not sure about half of questions and there were hardly questions which i said o yeah thats it i know it.
now i looked in MTB3 for internal medicine and god i hate this there are exactly those things that are missing in MTB2 like OTITIS externa for example etc. whatever i will revise arrythmias and cardiology tonight.
and how doing fast revisions: i am trying not to do less than 50 pages (i am starting this now only because before i didnot have time for MTB alot.
two three chapters at least. with one block of UWORLD explanations AT LEASSST and believe me i am not dumb but i have reviewd lot of questions lot of times and still saving them marked. i can not let go any ECK question those that are with dense dense explanations of one topics which are far better then reading any book.
okay guys sorry for being disheveled hope this helps some how
just try to build up in step wise fashion. and study more hours not less and just avoid all the distractors not only distractors just avoid everything avoid to do anything. everything is against study. even thinking about anyone . this forum is really an acceptable thing. does not take to much time. and we are all friends supporting each other eveyrone in same situation sharing experience. yeah and if it helps i have mobile phone turned off all the time. although nobody ever remembers to call me any more ))

its been a while since i posted something, but my schedule has got me running non-stop i totally understand ya tamta on cardio, but i gotta tell ya what helped me was kaplan qbank.... cardio questions there made me practically memorize all the cardio chapter from MTB and FA

NBME4 221, after 20 days NBME6 235, after three weeks UWSA.
i had one same question as NBME. i really dont know why UWS felt so special, maybe its placebo effect that we all know that it correlates more, maybe because i wrote it in library not at home, with real people moving around and disturbing my mind.
i made analysis today and i made at least 5 simplest idiotic mistakes. but on other hand i answered lot of hard questions and lot of questions that i though i mistakenly answered appeared correctly answered bu gut feeling. and generally this comes down to gut feeling, and only reason revisionis so important because we have to have everything ready in our mind to use that one minute just on picking answer not thinking about diagnosis and whats going on generally in question stem.

i had 10 incorrect in block on average. and i have marked 20 on average never had time or wish to review more than 5 though. so i think on exam its better to mark the qs that may really be doable. although hard to predict which will be doable and which not.
why it was hard: because it seemed like they have exactly known my week points exactly knew what i had reviewd in last days and what not and tested me on that, and also the softest topics they made it so tricky. and as conrad sais, god bless him: every incorrect q has something that you like about it or you would not choose it.
is it placebo or not i am so focused and calm today that even last evening reviewd one whole block of uw in two hours (old block), one new block this morning 2.5 hours, and did religious overview of some pharmacology and renal, neuro and hematology parts. now doing cardio.

Default Incontinences: Types and Treatments

Hello!

This is a small summary for all the incontinence and the possible buzzwords that can help you figure out the cause! I used FA for these notes.

p.s. thanks tamta for sharing your experience with us, you are going to rock the exam , and since you mentioned that you still missed some of these type of questions, i thought it would be of some help to post this although i think i do recall you posting one about it but oh well, the more times we read it, the better we get right?


TOTAL = uncontrolled loss at all times and positions.
= Mechanism: loss of sphincter efficiency (surgery Hx, nerve
damage, neoplasm) or a fistula between urinary tract and skin
= Rx: surgical repair


STRESS = due to increased intra-abdominal pressure (cough, sneeze)
= Mechanism: sphincter insufficiency due to pelvic floor
muscle weakness
= common in multiparous women or post pelvic surgery
= Rx: Kegel exercises, pessary, vaginal vault suspension surgery


URGE = strong and unexpected urge to void unrelated to activity
= Mechanism: detrusor muscle hyperreflexia or sphincter
dysfunction due to inflammatory conditions or neurogenic
= Anticholingergics, TCA's, Pelvic floor excercises (main therapy
when contraindications to the use of anticholinergics or TCA's
exist, such as acute-closure glaucoma Hx)



OVERFLOW = chronic urinary retention
= Mechanism: distended bladder that cannot contract and
fills up with urine until the pressure inside the bladder
exceeds the pressure of the sphincter resulting in urine loss
(dribble)
= Rx: urinary cathetirization (Acute management), treat the
underlying problem, timed voiding

why it was hard: because it seemed like they have exactly known my week points exactly knew what i had reviewd in last days and what not and tested me on that, and also the softest topics they made it so tricky. and as conrad sais, god bless him: every incorrect q has something that you like about it or you would not choose it.
is it placebo or not i am so focused and calm today that even last evening reviewd one whole block of uw in two hours (old block), one new block this morning 2.5 hours, and did religious overview of some pharmacology and renal, neuro and hematology parts. now doing cardio.
Does UWSA correlate more to the actual test? Bcz i thought NBME 6 is more predictive all along (since its the newest i guess)..I want to save the most predictive for the last week of January..I was thinking of taking one more in 2 weeks..So what do you suggest i do next?

sorry to barge in....
i heard that the 4th one was more predictive and also that UWSA was more predictive than any of the nbmes, im not sure how credible this information is, but for me i decided to do all the nbmes and take an average, that way i have a good estimation.

when is your exam?

I just scheduled my test on 1st february..Didnt get any dates for january, probably bcz there are no test dates in the first 2 weeks of january...I will do all the self assessment exams, its just the order in which i do them.

Uwsa is best i guess. I used to say that even before taking it. So save it for the last.
I also think that it depends on when y take assessment two or one month before or two weeks before. That makes big difference for me at least.

What i can say about uwsa so far it is very reliable above >240
But quite unreliable if scorre is <235 people got <220 on real deal.
This is from quite a lot experiences.Anyway that thread is great.

Yanichts thanks for incontinences this is really ridiculous because believe me or not i missed lot of qs on every assessment even now when i reviewd i thought i answered one correctly it was just by chance cause i misseddiagnosed type of incontinence and choose wrong answer for my misdiagnosed one hahaa so it came out correct. Such a shame..
I may not be sure about anything on exam but after so many mistakes i hope i wont miss any q on exam if there will be any about incontinence. Because for step1 i suffered myself in last days about acid base.. Really was weak in it and believe me i did not get any single q not even close to acid base. And whole my step1 exam as i remember was cardiology so.. Imagine it is same for me as incontinence
I would add that timed voiding and behavioral stuff is also for urge incointinence
Plus also bethanecol for overflow incontinence.

My exam is on 1st feb..I hope I'm still allowed in the january thread ..I will take NBME 6 in 2 weeks and UWSA on the 25th...I'm still not sure if i should do NBME 6 or UWSA first..Bcz i will be okay if the assessment exam shows a lesser score than overshooting the score..I get carried away too easily..As of now, Im doing the left over blocks on uworld (i know its taking long bcz i dont study during the weekends)..My q bank is going to expire on wednesday..So i will start a fresh subscription on thursday and do a subject or sub division from MTB everyday and do questions the same day..I cannot do MTB alone, gets too boring for me..Hope this keeps the interest cz Im disappointed that im not taking the test in January already..This is my plan as of now.

Did you take NBME 4 already? Bcz i think one should not skip the newer NBMEs...Im a big fan of NBMEs for some reason.

NO ST eleveation i.e. no clot --> YES HEPARIN prevents clot formation

need to do MTB TWICE !!!! no matter what. with UW questions of course. UWSa 4 BLOCKS TAKE LOT OF TIME AS THEY ARE NEW EXPLANATIONS. and while doing them i have referred to my step1 book much more times then during whole my preparation.
still put alarm on 7, opened eys at 8, got out from bed 9. i don't know what to do. its better to sleep until 9 then suffering like this whole day, but i need to start waking up early

yep its right! NSTEMI is usually precipitated by clots that are platelet rich, this is why heparin works and fibrinolytics dont. (i went back to kaplan videos with fischer for cardio, just to get it settled in my head forever haha)

p.s. tamta ive done all nbme's also, so i guess ill just go over them again offline, to at least get some questions done
finishing OBGYN today, and going on to surgery, got 2 days for review and to finish the subject based questions from kaplan qbank.

ah these last days!

How come vaginal bleeding is superficial type of bleeding.. Cause its superficial endometrial sloughing

just did 1 block of cardiology . Got 93 percent. Since I did it 5 months back , I think I only remembered 1 question and rest of the questions felt like new.

It means I know the concepts.there was one question I got confused. A woman in early 30s , presents with chest pain , though u understand it most likely emotional or psychological , still do u do ETT or just give reaasurance , I got similar q in USMLE RX which opt for further testing by ETT but World says do no more tests.

Step 2 ck is vague. But, we have to just trust yourself. appear in the test as much as we know. Nobody can know everything

Tamta ,thanks for keep us motivating. Was UWAS was all about playing with the concepts of the q bank. Did u feel like that some of the concepts are new .

Just give us a rough idea on what subjects they stressed on. which subjects need additional reading apart from the q bank and MTBs.

Have not seen my kids like for month, its wintrr holidays thats why i planned like this cause when its school and i have to go out it already masses up the day. bought train ticket yesterday sand kindof missed routine morningtsudy until lunch time.
So what i was saying that little kid is with nanny and once she called me that she was saying one night that wants to see me i see her twice or three times a week just for an hour maybe. So i went and took her for christmas night. It was around ten oclock i had some money so i thought would take her for a walk in mall (at 10 in the evening). And buy some toys maybe just for her to feel something like holiday. So we went there and i didnt even buy a salad for me and spent all the money on toys. There was one microwave she liked it but i said its expensive and she didnot protest at all. She was very happy with all the stuff we came home i thought she would play and i read at least half block tonight and suddenly she remembered that microwave and started why didnt you buy it to me i usually ignore when someone especially kids complains so she started repeating it and crying and etc. got so angry in myself that i took her from nanny because of her words that "kid said she wants to be with me" y know what everyone will want to be with us when we have nice job and nice salary.
And i am doing this not for toys and not for play and not for buying fancy cloths, but to give a good social life to my beautiful kids.
One professor told me "y are painting your future today"
Yeah thats what we do.. ANd how hard it was not we have to do this quicker, soonerand harder we work, more efficient we will finish this.
Woke ip Terribly hypotensive at 8 I'm glad at leas that helped me stand up early
So what about uwsa i dont want to spoil it for y guys and name the subjects just imagine that i missed qs about softest and most comfortable subjects for me bcause: they tricked me.. Because i was not sure although my gut told me correct answers. S i would say trust your gut and dont mark too much questions so that marking qs would make sense not just being a sign of panic and marking everything in a row.
Im so thankful to god, that i can take exam..
I don't care much about outcome already. I mean hopefully i get >230
Work hard guys.. I think i burned out myself. But im here continuing same hard study burnt out

93% in cardio after 5-6 months thats great believe it or not my cummulative is 75% only did around 110 blocks. And average blocks are as my highest scores on first rune around 82%.
And im telling y im not dumb
And im doing same qs with those large bulky tasty explanations for couple of times i a, using uworld to study reading it like a book.
Really this is a great tool. And i will not worry that i could not make it whole for second time and ta i didnot read kaplans well, and that i my have read step2 ck first aid blablabla...
I will passthis exam, i will be in my best shape on exam day (although im fat, these pounds are worth it)
I will not make stupid mistakes i will not get tricked by them, how many times i didnot read my weak and generally troublesome topics they will give me vague scenario and trick me on the softest subject and make me even miss qs in psychiatry.. So whatever
Main thing is exam day performance..
And i will burst out everything i have studied for and suffered myself relentlessly these couple months

I have so much trouble with pediatrics....does it come a lot in the real test? :S..what can i do to improve? And of the ones that are not IM, which is the most important one for the test?

Probably most subjects are fused in MI. I do them like together. Cardiology + cardio ped. Neuro + neuro ped. Hematology +hema ped.
I guss uniques stuff are congenital stuff, neonatal issues, ped surgery, dont know what to advice. I consider it same as IM.

Hepatitis treatment blahhh

So for hep C: Interferon ribavirin and either telaprevir or boceprevir these ares ame for acute and chronic
HelB: any of the following: lamivudine only one i remember, adefovir, telbivudine, entecavir, interferon tenofovir

Its getting harder to study whole day im really forcing myself and trying to hold on. Plan to do free 150 day before exam. And review of all charts. I cannot NOT do anything day before, it will be just light look over mtb. And i will rest whole evening. That will be more than enough of rest for me.
I miss my Yerevan my best colorful days are associated with it. God help me.
I will be obsessive compulsive stay in same hotel do same things as for step1 it same salad evening before. And have big vegetable breakfast in the morning.

Im so jealous of myself that this will be over in one freaking week

Yes very solid, so solid that need to buy some xtra large cloths to be able to go to test centre

What a wonderful day today after terrible whole day struggle yesterday.
After good night sleep, good life and with good mood and with perisstent review of MTB it is possible to do three chaoters like endocrine, nephrology and. Neurology until lunch time it is getting easier after each time and when i have done review of coupleof days before reading flows quite easily. Doing also 2 blocks of uw a day. Saving Those perfect explanation questions and marking them over and over again, those with lot of exhibits, lot of ecgs and etc. To have one perfect review random block in the end on day before exam

Work hard guys no depressions and psychyatric stuff. We will have for sure best year, best spring, best summer
Always remember we have done step1, i dont know about y but for me it was achieving impossible so no everything is possible for us.
And to tell the truth this last period for ck is much easier. Cause this is all about concepts that we have been working on these months. And they are already in our heads. Main ting is how we organize everything together. When y do that thats when y are ready.

so on the topic of crazy days, i just got notified that my exam on the 16th in italy got cancelled and my eligibility ends on this month....was forced to cancel my hotel reservations and flight tickets.. finally i booked a place for the 15th of january in berlin...so tamta ill be the first one to go!
nothing like these problems when you are in the last days of study
right now im just finishing my last 180 questions from the kaplan qbank and on the weekend will go over nbme 3 and 4 and just read and re-read the FA. my FA is annotated to a point where all i can see is colors

its been very hectic but you guys are really awesome for all the support we give to each other, doesnt make me feel so quite alone

hoping my mind will be set for the test day
keep studying hard everyone! we can do this! we can nail this exam!!

i just saw a post by fischer recently regarding new hep c drugs, maybe helpful to make a note and just keep them in our minds: simeprevir (NS5B inhibitor) and sofosbuvir

Only thing im afraid of is not to lose passport or not to have any technical problem or i can really go crazy.
Cant sleep
Waiting for friend from Ny tomorrow
Was looking at apartments on manhattan
From Tomorrow will go to library not cozy last days because of guests. So.. I dont know im kind of calm and probably this is just calmness before furious last days.
170 marked questions left didnt read much after midday. Hardly could make explanations of 30 qs from uwsa last block.
God bless us all..

You have to calm me down on 15-th when y come out no matter how hard it were not.
I thing we just need to keep ourself strongest on that day and we will be fine

I escaped and settled four nights before near prometric in yerevan very nice hotel

If explanations are new i. E doing first time 2 is my maximum
And ufind it not effectie i etter do one bock and read the other part of the day
IM doing same until now
On second time random all is easier
But marked subjectwise still takes same time an is hard
Even on second time no more than 3 probably doing only uw
i never done that

My four nights before sleep regime struggle has started.
Trying to sleep for more than two hours already i started striggle at nine in evening
This is new method. The earlier y start struggling with falling asleep the greater y have chance to sleep couple more hours

When i try to sleep im tting scared be ause that is the time when my mind does not work and starts to think about exam

Okay good night up from 7 tomorrow whenever i didnt fall asleep and ifnished late night studies after 9 im done.. Now priority is my well being only


One parent of US medical student calmed me down that this exam is easier they say than step1, and it old her yes if i had done rotation in every subject plus shelf exam after and in each one.. Why not would be so easy
Whatever guys we are doing great and to tell y the truth scoring above unbelievable is not the main thing i guess.. Its just about having one good interview, one good medical contact. Thats it. Everything comes down to that
So whatever....im a happy woman.. Finishing this..

So i dont think my sleeping pattern is interesting for antone but.. Was able to fall asleep around 2 am. Had spontaneous awakening at 4:50 b wasa ble to get back and sleep until alaram at 7;am and as able to get back after snoozing until 8:30.
This is torture.
I gues would be better just start fall asleep at 12 and sleep until 10 to be refreshed whole day and study at least. I am sleeping on uw blocks now.

Slept a lot eat cool breakfast did not study at all feels great okay just reviewed obstetrics reviewed in the very direct meaning of that word just turned oages and read highlights i cant read these same things anymore seriously im not getting meaning of eat so familiar that just readin like robot so i will do same kind of review of everything just turning opages and maybe i take assessment

thank you! yea i took nbme 4 twice (surprisingly i didnt remember any but maybe 5 questions from the first time that ive done it) and i havent taken any assessment in over a month and i wanted to see if im still in shape!

thank you rockstar88! well i was preparing for december actually, but i had problems with my permit and i had to delay until january, which only i guess worked in my favor (i managed to do more questions). believe me the nbme scores are shocking for me too, considering my step 1 was not the greatest, but i really tried to prepare my best for this one

Big snow this morning.
Im continuing my nice hotel stay.
Eatched russel peters today was fun. But i love robbin williams stand ups if anybody out there who can watch it and have fun my appreciactions.
I tried red bull first time now tot est it maybe i shall need it for exam so far nothing special, i am usually even higher
Woke up at seven and never slept back so good news already

I would strongly recommend weapons of self destruction by robbin williams stand up comedy

Why is it that even though i have about 2 weeks to the test, I'm not interested in studying? I have serious concentration problems..Does anyone else have the same problem??

Good morning everyone
As it is the big day already i could not miss to post it here.
Slept arpund 5-6 with two awakening and hardly able to go to sleep after being in bed since 9 evening, this is a good achieving i feel happy, i feel ready for something very hard. Listening to led zeppelin
Im glad that i will finally make this over today.
Thanks everyone on this thread and forum, everyone who just gave a glance what and how we are doing and struggling with our routine
Keep it up guys each day contributes
