Question hyperparathyroid investigation

the role of imaging in primary hyperparathyroidism

do we need to do imaging studies before surgery to locate the affected gland or only neck exploration is enough? (KLN mentioned both and caused a confusion).

According to UW you need to do a nuclear scan like sestamibi to check out presence of adenoma and its location. If you spot an adenoma, doing, minimally invasive parathyroidectomy under radioguidance should be sufficient. But if nuclear scan is equivocal or negative you'd have to go for neck exploration with intraoperative PTH level estimations.
