hullo, I have a question
Do bowel and bladder dysfunction present earlier in conus medularis or in cauda equina syndrome?
I think the answer was given in Uworld (my subscription ended )

Cauda equina bladder involvement is late but early in conus medularis.

Great going! How do you plan you go with your NBMEs? (And what was your Step 1 score, if I may ask?).

Just wanted to share, i did NBME 4 got 260. Felt easier the NBME 2 (237)
But questions are still very different in format from UWORLD or Kaplan Qbank!

good going!! congrats!
Resist the temptation to rest on your laurels though, I've been reading ck experiences and people say the actual exam is more difficult than both uwsa and
nbme, so lets keep our noses in our books!

Hi everybody,

Do you know what percent of questions you need to answer to pass? !

I need only to pass!

I am already an ophthalmologist and a fellow and I need just to pass to get my regular license here. i am very busy , do not have that much time to study . I will have one week off ! work to study compressive, I have done just 500 USW questions so far , average 70% . I may even not finish those, my exam is 12 days later.

I passed step 1 in the same way ! I had done 1500 questions I think .

Any suggestion?

I think you would have to finish at least 2/3 of uworld to be sure of passing, and at least 1 read of MTB 2. Beware though, MTB is severely lacking in Surgery, Gyn and Peds. Just to be sure, why don't you take UWSA?
Dont take Ck lightly, it's as difficult than step1, if no more..

Hey guys! Joining in exam in mid April hopefully. I wanted to ask I skipped Kaplan med and instead did MTB 2 and MTB internal med supplemented a bit with MTB3. For rest of the subjects did MTB3 and Kaplan. Was it too brave of me to skip Kaplan for med? I heard its so haphazard that u really don't get much idea about schematic management. Done with uworld once with 76% avg. planning to take nbme 4 in a couple days. Need ur input. Second thing was where should be study X-ray and ct MRI from?

Great going.....can you please tell the difficulty level of nbme 4 in comparison to nbme 2 and U.W bank ( nbme 2 is suppose to be on the easier side i guess ???) and how many questions you got wrong in your nbme 4???

Thanks in anticipation

Hey, well i only got 237 on NBME 2... i made many stupid mistakes. But i think NBME 4 was a bit easier .
For NBME 4 i did 17 mistakes with 260 Score, i dont about NBME 2 tho!
Good lUck

Well i dont think im the best person to recommend now, but i did Kaplan Ped, even thou its long and boring but after couple of reads it gets easier and its not that much. For Quick Review i use MTB 3 buts its missing TONS of info that is in Kaplan. So i would combine both!

Well....The thing is, during Step 1 i was ready to do it!!! But now, even with good scores on practice tests, i feel not ready at all. Even tho i read all the books 10 times and did 2 Q banks!! This test is not as easy as i thought it would be!!

Well I used
Kaplan Q bank...score about 73%...Good practice i guess. But really ....a lot of low yield Q's.
Doing 2nd round of Uworld....Much better Q's...and good explanations.

Sources:
IM-MTB2
Surgery-Kaplan/MTB3
Pediatrics-KAPLAN!!!!+ a bit of MTB3
PSYCHIATRY- MTB 3
ObGyn-KAPLAN!!!+MTB 3

How about you?

BTW//booked CS for End of July!!

ALL THE BEST ON CS...!!!!! Dont forget after share how it was..CS is the only exam im scared of!!

UW is amazing as always...i wouldnt say its hard or easy..somewhere in between. But it goes over every topic u have to learn i think.!!! Do it twice if u can to get that info stuck!!
And REALLY read all explanations and UNDERSTAND IT. But im sure u know the drill....u rocked step 1...u gonna ace CK

Doing 2nd round of Uworld....Much better Q's...and good explanations.

What ever you study; wont feel enough. Am done UW 75% still left with 25%, mind you this is my first time. I have to revise MTB 2 & 3 and do the rest of the questions. If I have time; I will do marked and In corrects. Did two NBME and missed 20 and 19 from form 2 and 4 respectively. I wont postphone this exam, will be done with you same day. The moral of the story; we both are different and did revise differently but still we feel; we have 2 read more. Let me be honest with you; just feel ok with what you have and conquer it.

Guys where do u recommend studying preventive medicine from? I just did nbme 4 and didn't do good in it Exam in around 3 weeks. Did worst in preventive. Dunno why I love to make stupid mistakes. One qs I actually read what is 'indicated' instead of 'contraindicated' and marked that wrong :'(*

Hey there mm! Prep's same as you guys. You can't be prepared enough. Finished Uworld recently with a cumulative of 80%. The most recent nbme for me was nbme 6 where I scored 248 (21 wrong).

Just revising the whole thing right now and plan to take uwsa after that. Have kept the last three days for a bit of casual extra reading and high yield step 1 content revision (some relevant clinical things).

Exam is on 9th april. Have my CS in mid May. Will try to take step 3 in/before August.

And man you surely are lucky to be in a med school that's cool enough to allow you to take your steps. I wish I could have been done with it in my med school itself but my school wasn't allowing enough.

Good luck to you

Ok , I am back guys
I took the test today ! I know it is not April just wanted to share my experience.

First thing: Don't panic! It is not difficult , just it is long and tiring.

Make sure to take snacks, food , if you are coffee person take with you , If you are a tea person like me take it! It helps a lot .
after each block I came out , I didn't do it in step 1 , but this time was better.

Questions are straightforward , you know the diagnosis , speciallay you guys that have studied hard, all of you can get high scores.

I am not a native speaker nor a native reader even my alphabet is differenet , but I could finidh all the blocks , so don't concentrate on that timer and don't look at it that much, just makes you panic...

You will do great

Hope mine comes good as well , I only need to pass though.

I didn't have time to read MTB! I did 1500 of UW , I can say all were in there just a little bit playing with the words or telling the scenario in another way.

Sometimes it is a long story you can diagnose in the 3rd sentence, look at the answers, sometimes you don't need to read all of the question.

Default Ok, the moment of truth.

Did the UWSA yesterday, scored 261, which is still decent considering how badly I messed up the second block. Was kinda saturated with the whole thing. All geared up for the real deal on Wednesday. Really drained up right now though. I think I'll take a day off tomorrow. Let's finish this!

Thanks man! Nah not yet. I might do the free 150 today (feeling too lazy right now, haha). NBME 3,4 & 6 were 244, 246 and 248 respectively, in that order; within a span of around 2 months.

Hope everyone prep is going well. I got a question regarding UW qbank. Ive been told your UW score should gradually improve over time. Ive done 10 blocks so far in timed, mixed mode. I don't see any improvement at all, the marks swing back and forth between mid 60s to mid 70s. When should I expect to see improvement? I read some of the posts from a couple years back, it seems reading MTBs doesnt really improve UW scores, then what does? really appreciate it if anybody can shed abit more light on this. Thank you!

I dont think u should expect massive improvement from 10 blocks. thats not even 25% of the Qbank.
Make sure u go over q's explanations and keep practicing. It should go up!.
I did Kaplan and MTB 2/3 and i think i those two books is enough info!
Plus 60-70s is not that bad

Sorry mate! Was lost in oblivion for a couple of days, away from everything USMLE, haha.

So I took the exam in NJ on 9th. Overall impression - I felt MUCH worse getting out of the center than what I did with Step 1. But this has been a common theme in the past few months and should be expected. The exam is longer and tougher than most of the material we practice with. The part which makes you feel bad is when you mark a tonne of questions and can't review it in the end, making you feel clueless at the end of it. I did not miss any questions due to lack of time though. I also had to guess a LOT more question than I did with Step 1.

The only thing I DO regret is I probably didn't give my 100% mentally (unlike Step 1). By the 7th block I was saturated and kind off dragging off with the questions; FORCING my brain to remain focused. It's probably because the exam has an extra block and has longer question stems than step 1. This might hurt my score but I don't think that was under my control.

As for the difficulty, the questions are difficult in terms of not giving us enough information to arrive at a plausible diagnosis. Like they often expect us to diagnose based on the clinical question stem and don't give much clues based on the "labs", which the Uworld usually did.

What advise can I give?

---Give extra effort for Psychiatry (probably the toughest)
---Give extra effort for Drug ads and abstract (do maximum questions)
---Do drug ads in the end
---EXPECT the exam to be tough. Remember, it's tough for everyone and the scoring is standardized. People scoring from 230s to 270s all say the same thing so it doesn't really matter. Just give it your best shot.

I did NBME 3 today and scored 253. That is much lower than my UWSA=265. Upto now I did NBME 2(253, 4 weeks back); NBME 4 ( 255, 2 weeks back); UWSA 265( 1 week back) & NBME 3( 253 today). I hope, I will end above 250; exam exactly in 10 days.

Thank you DNA. I have few questions regarding psychiatry. I was told to focus in differentiating different anxiety disorders with differentials and atypical depressions, bereavement. What was it heavily tested in your exam? Regarding drug add questions how many did in total you had? How did you manage your breaks in between exams and do you have a tip that would help us most? Thanks

Exactly what you pointed out. To be honest, I don't have a clue what more I could have done, maybe read another book or something. The description of the clinical case they provide in the question stem didn't seem to fit in a particular diagnosis. Maybe revise in detail the Uworld/Kaplan qbank Psychiatry questions, other than that, it's just that how good you are inherently with the subject that might help you on the exam day.

I had 4 blocks with drug ads with a total of 9-10 questions (I guess). I took breaks after every 2 blocks.

Another thing I might wanna add is do the free 150 questions EARLY (maybe right now.) The drug ads given in those free 150 questions will give you a much better idea of the level/type of drug ad questions you might expect on the exam day. (Even more so than Uworld.)

PS - I, unfortunately took that just a day before my exam. Scored around 93%. Easy questions overall but the drug ads are tough and challenging.

Thanks. Regarding psychiatry what was confusing about it? Were there so many treatment questions or differential diagnosis questions? What about ethics? I was poor in my step 1 score in ethics. Thanks

That's pretty scary :| hard for a person like u so it will feel super hard to us :| I have looked over the changes of DSM5 from Wikipedia. Dunno what more can be done for psychiatry :|

No I don't think the treatment aspect was confusing. It's just those "which is the most probable diagnosis" questions which I thought were tough. As I said the presentation didn't seem to fit a particular diagnosis. Ethics was much more easier than Step 1 and in fact I didn't have much ethics questions (as much as I had in my Step 1 exam).

@usmletaker - Relax mate. "Tough" is a very subjective word. Don't stress about it. The best you can do is religiously prepare and give it your best shot, and let your fate settle the rest.

I took the exam April 11th. I used kaplan psychiatry and First Aid for Psychiatry clerkship. Both were really helpful especially the latter book.

I am a 3rd year student in a Caribbean med school. I took the exam 2 weeks after my Internal Medicine rotation. I have surgery left to complete my 3rd year. My exam was more of internal Medicine. I used Kaplan lecture notes, MTB2 for IM both were really useful. My advice for you is that, ck is all about time mgmt, in order to save time, read fast, think fast and answer fast. Most of the IM qtns will have lab values. What I did which helped with time saving was to look at the lab values first by doing so sometime gave me the answer or removing the incorrect answer and then quickly run thru the qtn. For exam, I had two questns that I was asked the cause of hypoxemia. In those two qtns, the first thing I did was to calculate the A A gradient, the value from the A A gradient will guide me if its pulmonary or extra pulmonary that is causing the hypoxemia. Same thing I also did in the pictures, CXR qtns.
Hope this short review was helpful. All the best.

My Drug ads were more doable compare to abstract. I had 2 qtns from Drug ads( in sequential of 2 each making it 4 qtns in total) and 2 from abstract (in in sequential of 3 each making it 6 qtns in total) . I did not find the psychiatry hard to diagnosis. I got couple of qtns from delirium, dementia, like 3-5 qtns from substance abuse, tourette dx, depression etc

eren i took my test yesterday. just don't expect the test to be easy. It's nothing like uworld and the mock exams. just get a good night's sleep and don't be intimidated by the test. most of the questions you'll see will have atleast 2 correct answers and manage ur times well. i got short on time in 2 blocks. My test was super difficult but that's how i feel even though i'd be done with a uworld block in 40 minutes tops scoring atleast a 90%..but for the test i took yesterday, i am super scared. Ah well pray for me and best of luck

I have a mixed feeling. It is not as difficult as I have expected it, but a lot of silly and tricky questions.

1. IM generally doable but few rare questions appear but with classic presentation

2. Surgery are replica of UW

3. OBS-GYN most questions were about risk factor & pregnacy complications

4. Psychiatry a bit confusing but at the end u can crack most questions

5. Epidemiology and Bio statics tough but simple concepts; will trick in the details. If you know what they ask; silly

6. Abstract and drug add, I had around 9+4= 13 questions; paradoxically I find them doable and direct, they need only time

7. Ethics has always been difficult for me; may am unethical but better than step 1

8. I was finishing blocks at least 5 minutes before end and had time for revision

9. Took 5-10 minutes break after each block

10. Now I keep on remembering silly mistakes I made; as usual they will continue to pop up till the result come back

Around 70 % questions, I feel I did comfortably answered; Next 15 % may be 50 % chance; the rest 15 %, I have no clue what the questions were.

Step 1 I left exam center feeling I did terrible but end up in 260. This time I felt better than what I felt for step 1; may be the paradox may happen. Upto now, I have already counted 10 silly mistakes I made, poor me.

I didn't study last 2 days. Completely off. Just have a plan how you proceed with your exam and develop techniques based on your experience in step 1 and UW practice.

If it helps this is what I have listed as my exam technique based on UW practice and step 1 experience

What to take
Exam permit; not the one on the email
ID
Bag
o Banana
o Water plenty and chocolates
o Paracetamol
o Small towel coffee 7/11 or Gas station with sweetener
o Breakfast banana juice; no heavy meal
Meditate
Exercise
Praying
Keep silent
Dress easy cloth ; pocket less
Exam techniques
Drug ad and abstract
o Do them at the end
o Read questions and know what they ask
o Read the abstract/add heading and conclusion
o Scan to look for the answer
o Commit early
Simple questions
o Read what exactly being asked
o Avoid silly mistakes
Long questions
o Read first line and last line
o Know lab values by default
Time management
o Start fast and go fast
o Do in blocks of 10
o Usually I get stuck between 12 and 25/28; be fast on these sites
o Always answer even if marked
Marked and marking questions
o Come back when have time; know always the dilemmas; if confused go with the first hunch
Last three blocks when am tired
o These is where most mistakes made; be vigilant
o May be I may be weak between block 3 and 6; be vigilant
o Know every block is different
After each block
Wash face
Concentrate
Eat when hungry; toilet meditation

It is funny but this was my check list 2 days before exam; it helps to organize exam approach.

This is great! And so helpful! Thanks a lot. I think this is a very smart thing to do, instead of cramming till the last min, taking a step or 2 back and organizing yourself to be more efficient, so you perform to your ability when it matters most. Dealing with panic and anxiety will actually be more helpful for the exam rather than reviewing topics more n more....cuz this way, atleast we will be certain we dont lose points for stupid careless mistakes, or bad time management etc. Good job! Wish you all the best for your result!

The lengthy questions are usually easy to answer; once u know what they want to ask. The problem is I made so many silly mistakes in 1 to 3 line easy like but tricky questions.

Hello guys,
I took the exam yesterday. the worst thing I did before the exam that I didn't get enough sleep ( maybe 2 hours or less !!!))) before I went to my exam, I was thinking seriously to reschedule my exam and pay expensive fees rather than going to long-hard exam without getting enough sleep. I made a hard decision to go for the the exam but I wish I did not take it . I feel crap after the exam. during the exam I couldn't concentrate well especially on the last 3 books while I was having constant headache all the time. I'm concerning that it would be affect my score so bad

I hope the best for all and please get enough sleep before going to your exam

Same here; I barely get enough sleep but exam anxiety kicked me on. Hope it wont affect us much. I did get only 3 hrs sleep.

Just an update. Got my score yesterday and it was 258. So the all infamous script of "tough exam, feeling unsure after the exam, but getting the desired score" happened to be the case for me as well. And trust me I didn't even do full justice to my prep as I was almost numb in the 7th block. One thing I can guarantee you - the scoring curve is more lenient than anything else out there. So hang in there and keep going.
