Default CK in April

Hello. I started my prep yesterday. Taking the exam in the end of April. Started with IM. Passed Step 1 in August 2013. Kind of stressed out now because of the paucity of time as I don't think 4 months would be enough. But, I have no choice and have to do it.
Suggestions anyone? I absolutely cannot postpone my exam date.
Thanks.

9th of April it is for me. Done with Medicine, onto the minors. Got a month of observership coming up next month so that should take up some time. Here's hoping everything pans out as planned. 4 months should cut it @CMV.

Was hoping for someone to start this thread Good luck all.

Well Beka will give you more insight but surgery and Cardio sections are quite helpful ....you might consider peds questions as it is proven Achilles tendon for most test takers ..... In CK the more questions you do the better you will be as its your judgement call at the end of day ( Kaplan bank also helps you in retaining Kaplan L.Ns info)

Default MTB2 + MTB3 + UWorld

I am following the above forumula. Started UWorld and it is proving to be a fantastic teaching too. I am noting down everything in the MTB's.
Do I need to do Kaplan? It takes a lot of time for me to watch lectures and then read Kaplan and then come back to MTB.
Advice anyone? Please?
Thanks

PS - Exam end of April.

Default Behavioral sciences, help!

Hello all
I'm planning to do step 2 ck by the end of march. I took step 1 and was blessed with 258 . I did poorly in behavioral sciences though and am still doing badly, especially in the "What would you do " type of questions.
Does anyone have any recommendations on from where to study this?

kaplan bank was tough with long stem of questions and sometimes testing obscure points but it was good practice. I was able to hang around mid seventies but purpose was to practice and analyse stem in limited time....here is a useful thread for you

If you have not gone through "kaplan medical usmle medical ethics - the 100 cases you are most likely to see" then try to give it a go and obviously doing tons of questions is best solution for any problem in usmle

Thanks! I have not gone through this book but I will now.
I know a lot of people have recommended kaplan notes but I find they are too extensive and very low yield. For step 1, I tried reading all of the kaplan lecture notes but I really didn't remember much in the end. This time, I'm planning to study only IM from Kaplan LN.
What say you?

amm...

it's not essential for sure.

i felt it was good in pediatrics. otherwise if you don't have enough time don't go for it.

folks I got a question regarding the kaplan LN and videos, can you get away with just the video and MTB2、3 or Are the LNs necessary as well? And what are the difference between HY kaplan vs the regular videos like the ones at the centreprep? Thanks for your time.

Using notes or not is a personal decision really. If you're scoring satisfactorily in Qbanks then you're good.

HY videos are NOT the same as Center Prep. I haven't watched them but I guess they are more concise and less extensive. Maybe someone who has actually watched them would have a better idea. Good luck!

Kaplan CK Qbank Scores Thread
Thanks!

Yeah, I checked that out. I've finished around 77% of the Qbank so far and my aggregate right now is at 70%. Somewhat lesser than what I was hoping for but based on the many feedbacks I've gone through, I guess it's okay. And yeah some real solid practice no doubt.

You are doing great my friend....after kaplan you just have to top it off with U.W and hopefully we will have a good prep combo

If you are relying on MTBs then IM is fine(esp after you have gone through IM L.Ns once) but I would highly recommend to go through Surgery( clinical vignette section esp trauma part) Peds L.N(as MTBs are missing some topics big time). Do watch videos of Kaplan OB GYN as they are awesome (Dr Sakala has done amazing job)

I am abandoning Kaplan LN. It is taking too much time to go through everything. I plan on sticking to MTB2, MTB3, UWorld QBank, Step1FA.
Don't have the time man. Really stressed out. I hate to abandon Kaplan and am scared doing this, but I won't be able to complete the course otherwise. WTH

Count me in! I took step 1 a month ago and I hope I could get CK out of the way in April. I started test prep this week, I will be doing MTB2 and 3, Kaplan lecture notes, and Uworld. Do you guys think kaplan qbank is necessary for a 250+?

I've seen Drug Abstract questions in UWOrld, They show you an advert for a product that quotes the outcome of a study to validate its safety / efficacy. Then they ask you to interpret the statistical analysis of the study. Basically, Biostats.

MTB is the easiest, a step down from Uworld and all of Kaplan. If you've mastered the latter I don't think you would need the former.

Well the reason peds haunts most of test takers(IMGs) is because Peds KaplanL.Ns are badly written (less coherent text) and hence it remains a sore point........ but(according to past test takers) best way to study peds lies in our ability to consume(this terrible) Kap L.N since it is complete peds book containing all necessary info required for this exam so my advice regarding peds would be:

Kaplan L.N + MTB3 +Kaplan Qbank+ U.W Q bank.( if you are on real time crunch then you may skip kap Q bank but in CK the more questions you do the better your judgment gets and this translates into better score)

Buy it! you can get them off amazon. They are books you really need to learn inside out. Like FA was for step 1. Even though these books are not enough, i think you want to own the latest edition and make them your best friends.

Been hearing a lot about "Step up to Medicine". Any reviews? And is there really no need to buy First Aid after the MTBs. (Kind of a stark contrast from Step 1 where First Aid was everybody's favorite.)

although nbme 3 and 2 are old but I believe their score curves has been tightened to reflect present trends (to some extent).... that being said UWSA and nbme 4 are better predictors which should be taken at later stage of prep (esp UWSA ) but certainly you are on right track ....use this nbme to identify your weak areas and work on them ...good job and keep doing questions along with your study material.

(p.s can you please share your study material till now)

I agree Plan

started UW 1 month ago Timed,Unused Randome Mode, doing 1 block per day. explanations take huge effort and amount of time. going through every answer.

done with 31 blocks so far. going to do another 9 during upcoming 7 days so i'll be done with 80% of UW. taking 1 week off from UW after that. from 22-till 28 feb going to review MTBs, KLN GYN & OBs (worst subject for me so far) , Biostats... overall forgettable materials you know.

on first of March i wll take NBME 6 online if i score >230 will continue in the same pace. if i don't depending on performance will change the plan.

will be done with UW on 11 of march. and i will have ca 1 month to review Marked and incorrect ones. along with MTBs & stuff. + going to do DIT 2013 CK during that time. that will take around month. planning to sit for exam in last week of april.

Doing psychiatry and epidemiology from MTB 3 and FA step1. Tell you the truth; nothing is close to FA, everything you need is summarized there.

ok , you mean that we have to depend totally on FA for epidimiology , is it the same for biostatistics and have you done kaplan lecture notes

I did kaplan for step1; I don't think I will do so again. I may do kaplan questions on psychiatry and epidemiology than reading the book. That is my plan if I do perform poor on assessments.

Doing UW mixed and timed. Consistently low score on pediatrics. I have to revise kaplan while doing the questions. The first 70 pages of kaplan pediatrics are the gold mines.Hope everyone is doing great.

Default pap smear ?

So, everywhere I look I see different guidelines for frequency of Pap smear.
So far, what I'm going with is once every 3 years from 21 to 29 and thereafter every 3 years OR every 5 years if combined with HPV DNA testing
Any thoughts?

21 years or with in 3 years of first sexual contact;and you can stop it in those > 65 and normal paps in past 2 screenings.

Start Pap from 21, despite onset of sexual activity.
21-29---> every 3 years
30-65---> every 3 years cytology, or every 5 years cytology +HPV testing.
>65---> no screening if recent prior tests negative.
No screening---> if hysterectomy for benign causes (i.s. no previous history of precancerous lesions, cervical cancer, or DES exposure)

ASCUS----> 21-24----> repeat cytology in 1 year
>25 -----> HPV testing---> if positive do colposcopy, if negative- repeat Pap and HPV testing in 3 years.

MTB sais do repeat Pap in 3-6 months and order HPV testing.
Had trouble with this in question, cause they gave both as different answer choices.

SO it is clear that if 2 consecutive Paps show ASCUS, or HPV16 and 18 are found or ABNORMAL Pap ------> colposcopy and biopsies.

Endocervical curettage---> all NONpregnant undergoing colposcopy for an abnormal Pap
Cone biopsy---> if Colposcopy and ECC are not consistent (i.e. abnormal cells were not biopsied) , abnormal ECC histology, endocervical lesion, biopsy showing microinvasive carcinoma of cervix.

Observation for ----> CIN1, and CIN2,3 after ablation or ecision
Ablative (cryo, laser, electrofolguration)or excisional procedures (LEEP, Cold-knife conization) ----> CIN2,3
Hysterectomy----> biopsy confirmed, Reccurent CIN2,3

Pap in pregnants managed same way except ENDOCERVICAL CURETTAGE.
CIN/Dysplasia- pap and colposcopy every 3 months during pregnancy, repeat Pap and colpo 6-8 weeks postpartum. If persistent lesion definite treatment.
Microinvasive cervical cancer---> cone biopsy to rule out frank invasion, deliver vaginally reevaluate and treat 2 months postpartum.
Invasive cancer <24weeks---> radical hysterectomy + radiation
>24weeks---> conservative management up to 32-33 weeks, cesarean and definite treatment.

Classification of Pap Smear:

ASCUS----> intermediate smear
HPV, Mild dysplasia, CIN1---> low grade squamous intraepithelial lesion LSIL
Moderate, Severe dysplasia, CIN2,3, CIS---> High grade HSIL

Cancer: Invasive cancer (management: cervical bipsy, metastatic workup----> hysterectomy. if mets, tumor>4cm, local reccurence, positive margins---> Hysterectomy+ adjuvant therapy radiation or chemo)

HPV vaccination:
Quadrivalent HPV recombinant vaccine (Gardasil)---->8-26years
Sexually active women can also receive vaccine but in above age range only
not recommended for pregnant, lactating, or immunosuppressed women, also recommended for men same age to prevent Genital warts. Vaccinated ones should still follow with Pap smears.

That is really a good score. You have ample time to work on weak areas. How many questions did you get right?

Compared with the time you have almost 6-7 weeks; your result is above average and you should be proud and work on the weak areas. I tell you that is a great result.

Todays high yield

There are four combination: DTaP, Tdap, DT, and Td.

DTaP and DT are given to children younger than 7 years of age
Tdap and Td are given to older children and adults.

DTaP: Children should get 5 doses (2, 4, 6, 15-18 m and 4-6 y).
DT does not contain pertussis, and is used as a substitute for DTaP for children who cannot tolerate pertussis vaccine.

Td: adolescents and adults as a booster shot every 10 years, or after an exposure to tetanus under some circumstances.
Tdap is similar to Td but also containing protection against pertussis. Adolescents 11-18 years of age (preferably at age 11-12 years) and adults 19 and older should receive a single dose of Tdap. Women should receive Tdap during each of their pregnancies (preferably in the third trimester between the 27th and 36th week). Tdap should also be given to 7-10 year olds who are not fully immunized against pertussis. Tdap can be given no matter when Td was last received.

Upper-case letters denote full-strength doses of diphtheria (D) and tetanus (T) toxoids and pertussis (P) vaccine.
Lower-case “d” and “p” denote reduced doses of diphtheria and pertussis used in the adolescent/adult-formulations.
The “a” in DTaP and Tdap stands for “acellular,” meaning that the pertussis component contains only a part of the pertussis organism.

Hello all!
I just took UWSA yesterday and got 262!
have finished one round of Uw (finished only 75% of questions with average of 78%), then Kaplan LN ped and oga. Got lower scores in surgery and Peds, will do surgery LN and revise peds; also planning on doing mtb 3
I had started videos but found em too time consuming so stuck to the books only
planning on taking nbme next week
keep the faith!!
