Default Very frustrating questions

Finding it very frustrating going through kaplan qbank at the moment. Its really annoying trying to decipher what the question writers are wanting you to answer, even if the diagnosis and overall management plan are obviously clear....eg in "what is the most appropriate next step" type questions.

One question for example... in real life any 68 year old patient with severe abdominal pain, raised white cell count and PR bleeding is always going to get an abdominal X-ray in the initial work up, even if they have lots of risk factors for mesenteric ischaemia, and even if that is what the initial diagnosis is thought to be, way before they get an emergency mesenteric arteriogram. Its much quicker to obtain, and provides some useful information while an arteriogram is arranged. But its not the "most appropriate next step" in the world of step 2ck.

Which to me is a load of nonsense.

Some other questions seem to be the other way round also, making it impossible to second guess what the right answer is meant to be.

Another example.... a high speed RTA trauma patient in shock with abdominal pain and distention...clearly needs a laparotomy, but the question is "what is most appropriate next step" after only mentioning that chest x-ray is normal and vitals show he is in shock

Answer is apparantly emergency laparotomy and not spine films...doesnt c-spine come after airway in the ATLS guidelines!? No way would a patient like this go to the ER without having c-spine x-rays. How are you gonna intubate / move the patient onto the operating table without clearing the c-spine?!

A loada crap! Is the real exam gonna be like this?!

Sorry but i just gotta vent it out right now, this exam is annoying!!

I know. It's frustrating. Take everything that you've learned in practice and trash it. The examiners are looking for what they want. At the end of the day, this is a test. Try your best to learn the answers that they want.
