ObGyn Pregnancy and mania

What do you do when a pregnant women on Li comes to you? Li definitely causes Ebstein anomaly, but its frequency, even if pregnant women takes Li during first trimester, isn't high. Anyway, what to do- discontinue Li or not? And what after stopping Li?

Correct Answer

Stop Lithium. Treat with ECT for 1st trimester and Lamotrigine for 2nd and 3rd.

Li is absolutely contraindicated in pregnancy. one is required to taper and stop Li in a pregnant woman. I remember reading about use of lamotrigine and ECT being relatively safer http://ajp.psychiatryonline.org/arti...ticleID=176724
