Default Just did the test, my thoughts

Thought id help with a few things which i was unsure of before doing this exam.

First off though, I had the WORST possible start to the exam by only getting about 45 mins of sleep the night before. Just couldnt sleep...by 6am i was seriously considering cancelling it, but just couldnt face going through all this again, so went ahead and did it. I honestly dont know what kind of impact having this amount of tiredness had during the test, i think the questions i found hard i would have found hard anyway (apart from maybe the last block), but i guess will just need to wait to see what the score is!

About the exam itself:

The question length was NOT longer than uworld (or at least not noticeably so), they are very similar in length and amount of info to the medium / long questions in uworld. Some questions were quite short, as in uworld. I think the reason a lot of people report the questions being much longer is because the default text size in the exam is bigger than in uworld, so the questions tend to fill a lot more of the screen, but there really isnt much more info there.
I had a look at the font sizes in uworld, if you want to simulate the exam just change the font size from normal to medium

The psychiatry questions were by far the longest, but not impossible, just know the differentials for major depressive vs adjustment etc etc

Overall the question difficulty was a similar mix to uworld I thouht, with maybe a slightly higher proportion of medium vs easy compared to uworld. I actually thought that in terms of pure difficulty it matched the NBMEs fairly well, but the question style felt quite different.

I finished each block with 5-10 mins left to review marked questions. On blocks with a drug ad / abstract i left these to the end - these questions defiantly can take the most working through so id recommend doing this. They were of a similar difficulty to the ones in uworld, and i think if you know the biostats from step 1, and learn from the uworld questions, that should be enough prep for them

NNT and NNH are recurring themes in stats on this exam it seems, make sure you know this. I had one stats question as part of an abstract which was the only true WTF question on the test and had to make a pure guess.

At least 5-6 questions in each block came down to a 50:50 between two choices. You could make convincing arguments for why each choice was the one to go for. This was especially true for some pathophysiology questions which asked for example for the "underlying cause" for something, and gave 2 things on different parts of the pathway for a disease etc, or questions asking between two investigations or treatments which both seemed equally important. Now i dont know if sleep deprivation made this harder than it should have been, but it seems to be par for the course for this exam, and lots of other people report similarly. I think knowing this helped me just to go with what my gut feeling was and move on.

Overall the exam wasnt quite as bad as i had expected it to be (apart from being tired as hell)...but i guess that means nothing till my score comes back!

I used first aid for step 2 as my main book, did kaplan Qbank once, and the bulk of my serious studying from uworld. I think FA for step 2 is under-rated...its much better to MTB, which i gave up on halfway through. The vast majority of material on the exam is covered well by uworld, using this alone and having a good foundation knowledge from step 1 would probably suffice for a good score.

Also, ive just been checking up on a few of the 50:50 type questions...a common theme in the "what is the most likely diagnosis" seems to be that the history or examination goes with one answer, and the investigation goes with another answer. So which one you are meant to go with is anyone's guess...

The ones were i went with the investigation results over history i think ive tended to get wrong, so go with history and examination findings if you are truly stuck between two options i think

which subject had more twisted wtf questions besides biostats ?

Yeah if i were to do it again i might be more inclined to get a sleeping pill to make sure i got some damn sleep.

No one subject was particularly more difficult than others, as I said it was mostly the ones which came down to a 50:50 that were the hardest, and these were spread out over all subjects.

Many of the "next best step" questions were in OB/GYN, some of those were tricky make sure you know this subject well, esp the pathways for investigating amennorhea / DUB / etc etc

The images that i can remember were pretty straightforward yeah, had maybe 1 or two per block. ECGs were all fairly easy.

Update: scored 276. So a reassurance to anyone who is worried about not getting any sleep the night before! My advice, if you know the stuff and have been getting good nbmes, just do the test regardless

thank you for giving me a relief. I went through the same situation when I just slept one hour before the exam. I was thinking seriously to postpone my exam, However, I decided to take it. I don't know I feel like this might have affected my performance overall but who knows!? let's see how much I will get
