Question A young man with fever, sore throat and rash

A 30 year old single man presented with a week history of fever, sore throat, maculopapular rash on trunk and cervical lymphadenopathy. Only pastmedical history comprised of treatment for urethritis 2 weeks ago following a trip abroad. The patient has mild anaemia, lymphopenia and thrombocytopenia. This is his blood film.

What is the most likely diagnosis?

A) Mumps
B) Rubella
C) Drug reaction
D) HIV
E) Leukemia

I think drug reaction mostly by SMX TMP
or penicillin used in tt of his urethritis....

What do you think of peripheral blood film?

most likely it is leukemia, with hix of infection(urethritis), and pancytopenia

Blood film shows atypical lymphocytes which is the clue!

yes this is definitely leukemia -

is this the "ballerina skirt apperance" though?? im confused if that was mono or CLL

Drarnab,

Surely leukemia is a differential dianosis where a typical lymphocytes can be found in boold film.

This actually is a case of HIV seroconversion!

"single man" "Urethritis" are clues-suggesting towards sexually transmitted diseases!
