Default GU Question

A third year surgical resident while scrubbing in to prepare for surgery suddenly cries out in extreme pain, falling to the ground complaining of left scrotal pain. What is the likely diagnosis?

A) Variocele
B) Hydrocele
C) Testicular torsion
D) Epididymitis
E) Complicated ruptured scrotal hernia

Why not E) since it is not adolescent men and there is pain severe enough for the person not to be able to stand?

no. your best answer in this setting has to be Testicular Torsion.

- Tenderness or sharp pain in the groin often aggravated by lifting or bending.
- A tender lump in the groin or scrotum that usually disappears when you lie down, and enlarges when you cough, sneeze or strain.

if this was in the history i would say ruptured hernia. but giving the sudden incident with out warning as MedEX suggested, the most likely dx is Torsion.

When one is scrubbing in, isn't one bending over and isn't bending over a trigger for a potential hernia to rupture?
