U study once a week?

U dont need more then 5 months imo if u have a good dedicated study, even that is a bit to long.

Did u do Step 1 yet?

I believe your solution to your problem is setting a DATE to take the exam it seems obvious you haven't which made you Targetless

Why? Why start the journey just to stop in the middle of it?

Your step 1 score is good

And dont worry you are not alone, I also been preparing for over a year, but I ran into health problems (had a herniated disc which needed surgery so it sidelined me for more then a few months)

I lol'd

But its better to take your time if your practice scores are not up to par then to take the exam and fail (which is a death sentence!)

I think the problem might be passive studying. I have similar problem with step 1. You may need to find a way to recall concepts more easily. Ithink this is the problem...
