GIT Should we scope over 55 year old with GERD symptoms?

If a patient that is older than 55 years old in age came with new onset of GERD, do we scope?

In other words, do we consider GERD symptoms to be some sort of epigastric pain? because if a patient came with epigastric pain and he is older than 55 (or 45 according to Kaplan) we must scope him; does the same apply for GERD symptoms?

Yes.. It is very much necessary... The older the patient gets the more imp it is to scope him

This step 2 is driving me crazy. i hope questions are not crazy on the exam

you mean we should go through upper endoscopy immediately without consider the pain with food or after food or PH manometry 1st ,or even a trial of PPI ( ANTACID) ???? please need explanation

i know that because of age high probability of cancer but i thought we should go as usual .

ooh OLD AGE greater than 45 year , dysphagia , odynophagia and bleeding with GEERD considered alarming sign that need endoscopy and biobsy

DOTW, Please don't stress too much about minute conflicting details (to be honest, it makes me anxious just reading your concern (yes, that's my own fault...)). The reason I say that is that all of the questions on my CK exam were unambiguous. I never felt like they tried to trick me on something silly like 45 vs 55. (Yes, Kaplan does, but that just helps you to learn and remember.) Whenever there was a question where details like age or length of time mattered, they would make it completely unambiguous. For example if the cut off was 45 or 55, then the patient would be 30 or 70. Or if something was considered chronic after 3 or 6 months, then the patient would have had it for 1 month or 9 month.

I got a sense that the USMLE question writers are aware of where conflicting guidelines exist and they made a point to write the questions such that the answer was very clear if you have a sense of the general guidelines.

It is important to consider that any patient of any age group having symptoms of GERD for more than 5 years should have endoscopy
